Do you think it would be too harsh to add the mishap rules to all UMDs?

That would depend on the particular mishap rules, but I would ask the group how they feel after they saw the rules. I don't think giving the players one more thing to worry about adds much to the game, but some people may enjoy it depending on how harsh/light the consequence of failure is.

It is a very Low Magic game so Magic is kinda scary and wierd. Was mostly just ruleing that it does 1d6 damage per level of the spell being imulated for charged or use per day items. That type of a thing.

I think that is good enough. If it is low magic I am assuming they won't have the means to recover from a really bad mishap.

Silver Crusade

I am allowing some healing and the cleric does have scribe scroll. She is 3rd so cure mod scrolls will help.
