Has anyone converted the 3.5 AP PGs to PF? Is it a fairly straightforward conversion, just line up the new skills, or does it take a little more work than that for balance?
Generally, this is one of those "put as much work into it as you want to" things. I'm running Rise of the Runelords in Pathfinder, just mentally calculating CMB and CMD when appropriate, taking the average of Spot and Listen and turning into Perception, adding a feat or two, and adjusting NPC/enemy character class abilities as appropriate. I'd take the time to rebuild major bosses, but that would be about it.
Paizo released a 3.5 to Pathfinder Conversion Guide as a freebie that's handy to keep around for things that come up, such as negative levels, altered spells, poison, and so forth. But overall, I generally do such things on the fly.

I have been running RotRL for the last two years and it has run smoothly by doing the following: CMB/CMD, and skill equivalencies on the fly, and giving monsters max HP. On occasion for a big fight I may add a couple extra cronies, but nothing too major.

Which elements of the Player's Guides are you referring to? The equipment? The campaign traits? The feats? The prestige class? Some or all of the above?

This. Many of the items have already been updated to PFRPG, I'm pretty sure a good number of the feats were as well.
