You've seen it a almost every fantasy-based movie/tv show with a big monster. The hero climbs up the monster and does something like poke out its eyes or cut off its head.
And this is exactly what my players try to do everytime they encounter a monster bigger than themselves. Ogres, Dragons, Purple Worms, you name it, they wanna climb it. And I always find myself at a loss for how those rules would work.
On a slightly related note, how do you actually kill a giant upon whom your attacks will hit their ankles, or the monster whose tentacles are the only things characters can hit? How do you see it play out?

well: I would suppose with climbing a larger than you style critter you should grapple, followed by a climb. That said, grappling the larger foes is pretty difficult all by itself.
So I guess: establish a hold (as opposed to a grapple). You make a CMB vs the opponent's CMD and you enjoy a +5 to the attempt (As usual, it's best if you have oth hands free). If successful, you gain the grappled condition (your opponent does not)from there make a climb check (as a move action).
To your second question: Fights are dynamic things, with jumps and rolls and a lot of movement. Big creatures have a lot of HP so the first few strikes are bviously jabs to knees and hips. as HP start to drop, the blows are raining down on a monsters shoulder as he slumps, or when he bends down to strike.
As for the tentacles, well they are usually entered into as the creature's reach. IIRC you cannot actualy strike them, but have to move in. (Which seems silly..why can you not hold an action and strike the tentacles?)

Climb Aboard: move adjacent to a foe at least 2 size categories larger than yourself. In the following round make a DC 10 climb check as a free action to clamber onto the creatures limbs or legs(you move into one of the creatures squares). The creature takes a -4 penalties on attacks against you. the creature can try to shake you off by making a grapple check opposed by your climb check. If the creature succeeds, you wind up in a random adjacent square.
Special: fighters can take this feat as a bonus feat.

In our games, the player's mini is shown to be adjacent to the giant monster, but he is described to all the players as being all over the place, very much like you describe or that is seen in the movies.
Or do you actually think the ant is simply standing their trading blows with the titan?

Right I agree, the character is not just stading at the feet of the giant slicing at his big toe. Its all about how you see the fight and how you describe it, sure the dice might say hit just like any other, but in your mind perhaps the character spring boarded off the arm of the giant (as he swung) and thrust for the creatures throat. It gets even more exciting when a threat to crit, or a crit occurs... then some outrageous descriptions should be in order, and if you play with the three nat 20s in a row insta kill rule, some spectacular scenes have occured (like the decapitation of a dragon, as the hero rides its back!)
Sure in game turns it was just a roll of the dice and an attack, but in your imagination it can be much more, that is after all why we play this game right?

Had a Pc who did this all the time. He was a tiny (Perm Reduce Person) Halfling rogue with a cloak of spider climb and boots of Stick (3.5 spell of same name), but anything bigger than medium he attempted to get on its back. Gm made me make balance checks to stay on top of the creature and I took allot of falling damage with that PC but It was allot of fun the first time I asked if I could do it. Guess you could make them grapple or use climb checks. I, this was the other GMs rule as well, would not let them climb anything less than 2 size categories larger than they were.

How does a tiny size creature be at all effective? You have no reach and deal crap damage with your itty bitty weapons.

Getting onto the opponents back and sticking the itty bitty weapon into his jugular (Sneak Attack)?

sneak attack doesnt scale with size. a tiny creature can deal mad damage if it can sneak attack.
