Has anyone tried doing this? And if so, how did it go? Do you have any suggestions for someone who is about to run a pathfinder game using this medium?
The reason I ask is because I am about to start a Pathfinder campaign set in my home brew setting on July 6th and wondering if anyone has any pertinent advice they'd like to offer.

For a while, I was heavily into MapTools. It's a great free community developed app that allows for online play. At a basic level, it works well as a white board and map sharing app. At it's most powerful, you can use macros, fog of war, states, auras, character sheets, and all sorts of user-created tools. The only drawback is that the learning curve increases the more you want out of it. The app is constantly being pushed forward by the community. Here is the RPTools home page: http://www.rptools.net/
We use Teamspeak 2 and 3 for VoIP (also a nice app IMO). There are quite a few free servers that can be found, so we did not even need to set up our own.
Overall, it worked fairly well. The only hurdle that we sometimes hit is that the DM and players need to be a bit more organized. It's funny what you loose when you are no longer face to face.

It works fairly well. I often find myself having to work a bit harder than with a flesh and blood gaming group to keep them interested. Smaller groups work better with this, so 3-5 people is your sweet spot.
Organization is key, and fights will generally not take as long as they will in person. That could just be my group, though.
