I have what will seem like a noob question but I am having one of this duh moments. It states that the tatzlwyrm can rake when it pounces. Is rake a 1D4 + strength? what is the to hit on that? If it is listed then I am blind and appologize. Thanks for any help.
Rake is still one of those things I flip to the MM glossary about. (I dunno, just never sticks. No reason.)
Rake attacks usually are listed in the creatures list of abilities, but this must've slipped through. Fortunately, it's in the creature's Special Attacks. As far as I know, a creature can make a rake attack whenever it successfully grapples a creature. However, this critter is described as being able to make two rake attacks when it charges.
The way I interpret this is: If a tatzlwyrm charges, it can make a bite and two rake attacks. (The rake attack has a +4 to hit, and does 1d2+1 damage.) Otherwise, if a tatzlwyrm hits with its improved grab, and successfully grapples, it can make a bite or two rake attacks. In following rounds, it can use its poison breath, or bite, or rake attacks. I might be wrong on this, but this attack routine should be reasonable.

the way the creature is written worked fine when I ran it last Saturday. I was not aware creatures could do a full attack while grappling. Something for me to look into.
I don't believe they can, unless they are only using that appendage for the grapple, thus taking a -20 on a grapple check. Even then, they can only use remaining attacks to attack with.
The tatzlwyrm seems funky, in that its rake attacks sound like they should be claw attacks. But, by definition, as a rake attack, they can make them after successfully grappling a foe. Hmm...maybe it's "rake" attack was a misnomer? Unlikely, but I'm curious if there's anything I'm missing.

The rake is 1d2 + 1/2 Str...so 1d2+1...and the attacks are at +4. It's listed on the middle column at the top of pg. 15 in the module as part of the tatzlwyrm's full write-up in the appendix.
Also, instead of referencing only the text for a "rake" special attack, I think you should check the text for a "pounce" attack, as well.
Pounce: When a creature with this special attack makes a charge, it can follow with a full attack — including rake attacks if the creature also has the rake ability.
This means there's no grapple required to execute the rake attack if a pounce is involved. And, it can use this full-attack ability only after charging.

Note that it can only rake in a grapple if it starts the round grappling. DMs get that rule wrong fairly often, probably because the only place you find it is in the back of the MM, under Rake.
