Hey Folks,
I created a custom Scenario Reporting Sheet for my needs and figured I'd share it in case someone else finds it useful. This came about as a result of faction name changes (and a merger). Almost all Scenario Reporting Sheets are now out of date. I also included fields to help me determine APL and to quickly break ties for Initiative results.

I had an after thought to include a check box for slow track characters, which is something I will implement in the future. Please let me know if you have suggestions for improvement.

I like this quite a bit, and will be using it when I GM private events for sure!
However, as an event organizer, I like to be able to have multiple sign-ins per piece of paper used (the current reporting sheets are perfect since we often have 3 tables going during a game night). Would it be possible to maybe do a version of that with your awesome changes?

@ CanisDirus: When you print it you can choose a setting that lets you put multiple copies on one page. Depending on my event I will want 1 sheet per page or up to 4 sheets per page, and the printer function is all you need. Quite handy for many things, actually.
@xebeche: I spotted something inconsequential. You have a blank for a PC's dexterity score. I'm unsure why, I have to admit. But I can see where having a PC's constitution score handy at a glance would be nice to have.

Would it be possible, after you have made the changes, to post the word or the source document, in addition to the .pdf? For example, I don't find the inclusion of those various ability scores necessary in my games, but I might want to put something else in those slots.
Just a thought - I certainly like the sheet.

Xebeche, like I said, I really like yours, so I took Drogon's advice...although my copy of acrobat stinks, I used some of my photo manipulation skills (SO useful for teachers and GMs alike) and your base form and made the following. Thanks for creating yours - it's perfect for what I need locally in the below format! :)

This is a mistake on my part. I have been under the impression that if two creatures tie on initiative, and have the same initiative modifier, then their Dexterity ability scores serve to break that tie. After reading the section on initiative again I see that I have been mistaken!
If two or more combatants have the same initiative check result, the combatants who are tied act in order of total initiative modifier (highest first). If there is still a tie, the tied characters should roll to determine which one of them goes before the other.

For those interested, I have made sweeping improvements to my reporting sheet. As I have intentions of collecting more character data for individual tables, I haven't formatted my sheet to record multiple tables. Also, I use Apple's Pages application and haven't attempted to export to Microsoft Word at this time. As such, I've only included a link to a PDF.
