I thought the sneak attack damage could only be applied to a single attack per round...
You may be thinking of 4th edition, where this is the case. Unless otherwise mentioned, Sneak Attack damage applies to all attacks on targets who are either denied their Dexterity bonus to AC or are flanked by the character with Sneak Attack.

I haven't seen this mentioned, and if it has I apologize, but thrown weapons aren't ammunition. Shuriken are special, but that's just throwing away treasure at that point because they get broken. He still had to pay for two sets of +2 throwaway weapons, which is not reasonable for an 18th level character. Any NPC with well over PC gold, traits, a ridiculous stat buy, and perfect knowledge and prep time will in fact TPK a party. I can do it with a sorceror, fighter, rogue, or anti-paladin, though the fighter would be trickier than the other ones.

Yep which is why Two Weapon Fighting is a popular fighting style for rogues since it give them extra attacks to squeeze out extra goodness from that sneak attack damage.

Bane abuse kills players? You don't say.
I take issue with both poison use and throw-away items. I find it absurd that anyone can base themselves off the premise that they are effectively throwing away several thousand gold in an encounter.
