What is the difference between "uses ... a move action" and "leaves a square during a move action it's using"?

The former includes things like standing up, which is a move action but doesn't involve leaving a square. The latter includes things like moving past you when the enemy started moving outside of your reach.

Often (usually?) both triggers are met by the same action, but the text makes sure to include everything that could be missed by one or the other.

What if it leaves a square but remains within your reach? Or is that only in 5e that such a move doesn't trigger?

You have it right- leaving a square but remaining within reach is still leaving a square within your reach. It provokes an AoO in this system.
