When player characters reach a higher level, especially ones that use distinctive tactics, weapons and/or spells, it makes sense that they themselves would be the subject of much speculation within the world. As such, would NPCs trained in, say, Knowledge Local be able to make checks to identify PCs? Something like "The dwarf with the 4 pronged beard is Drangvos of Torag, slayer of [Insert Infamous Creature from character's career]. Specializes in divine self-buffs and melee combat, keep distance." Or "Human Male in the robes is...Sednis Morrigna Blakros. RUN."
Or would this be a vanity thing someone would drop prestige on?

I've seen some GMs run with it, and others sort of leave it by the wayside because the things one is encountering are effectively at the same 'Fame level', so it's a bit of a wash.
That being said, if I've gotten boons that note a title for a given character, I incorporate them into the character's introduction and have a small splat-sheet so the GM can quickly look over them and see if any of them have a significant meaning in regards the scenario...

There are scenarios that use this sort of thing, but they're few and far between from what I've seen. I think it makes more sense for the majority of NPCs to make their decision on observation by saying that they've been too concerned with their own machinations. Additionally, I think there's region-specific restrictions to bear in mind. Duergar in the Hao-Jin Tapestry, for instance, probably shouldn't be able to make Kn: Local checks against specific people from outside the tapestry.

Knowledge (local) doesn't give you the interns name. It gets you:
Base: That is a human. Humans can do this.
Little Higher: That human is a pathfinder
Little Higher: That human has druid levels
Little Higher: Oh s$%@, that is Osprey, you have heard of him.
With the intern, you probably never get much past the first two.
