Hmm, our phone number is 425-250-0800 so it sounds like it is misprinted on the certificate. Send us the claim code at customer.service@paizo.com and we can investigate what is going on with it. It would be helpful if you could send us a picture of the certificate as well.

I also see the misprint on the phone number and am letting the appropriate folks know so we can make sure that gets changed.
