Type #2:The IC is IC, OOC is OOC game. This is a game where people envision a world and then do what their characters would do. Regardless of whether that hurts other PCs indirectly. A game where it's not appropriate to trust or bring someone along simply because they're a PC rather than an NPC.
I think this is key. Not only does there need to be a consensus among the players as to the type of game they're playing, but there needs to be a potential for fallout when a PC acts against the party's interest. If someone constantly gets the party in trouble, but is immune to consequences because they're a PC, it's going to cause a lot of bad blood among the players.

They can't be immune, but others can optimise their characters to deal with it. If they are building aroubd a sword of berserking, every one else builds around getting out of range or stopping the rage. Deathwatch on the toughest enemy is also good.
Being afraid or hating one creature is ok as a character trait, but if they chain themselves to a die roll, you are in trouble. That's why I don't tolerate Kender. They are one die roll from causing a TPK.
A paladin that hates undead can make a good "bad cop" in an interrogation. They will let the ghoul go, only if they can find the vampire lord. If the stupid ghoul runs to warn the vampire lord, both just opened themselves up to scrying, and another shot at wiping the entire undead infestation from their God's beautiful world.
