15 ft. Cone template, above and below includes what?

Since conicals are 3D shapes in reality, I would like to know which spaces above and below a cone (say burning hands) are also targeted.
Is the "M" in >>>this image depicting some possible interpretations of what spaces a conical template covers<<< targeted as well and why?

The trivial answer is: none. PF doesn't use cubic spaces, only 2D squares.

Somewhat less trivial: mathematically cones can be defined for any amount of dimensions, not just 3. So from that perspective, there's no need for any cone to be specifically 3D.

This is somewhat borne out by a narrow reading of the CRB as well:

A circle is very much a 2D object, otherwise it wouldn't talk about a sphere.

I treat them as 3d, modified X- config, setting the widest point at ground level, which gives you a profile similar to this looking directly back at the caster (with ____being ground level)
