When I've seen it roleplayed, they only did so when describing what they do on their turn...so not that annoying, really. I've never actually played a Witch (I almost never play full casters), but that's probably how I'd do it.
Anyone who laughed through other people's actions would be quietly killed and buried in the backyard. For the good of us all.

I enjoyed roleplaying my cackle as a constant stream of encouragement on my hopelessly optimistic and extremely emotionally supportive buffer witch.
"I know you can do it!"
"Wow good job!"
"I believe in you!"
"Nice swing! You showed him the power of friendship!"
And now Fortune has been renewed. I wish that game had lasted longer than a couple sessions.

Wish they'd fluffed the Witch's Cackle Hex as the Shaman's Chant Hex in the first place. Given that they didn't, the ACG should have made the Chant Hex available to Witches ("this works like Cackle, except that it is often less annoying to the Witch's allies, thereby decreasing the chance of being eaten by one's comrades . . . .")

My wayang witch was played as extremely creepy and sort of the thing of nightmares, and has a unique high pitched sort of breathy but low volume voice. No one thought his cackle was creepy. Though a few members of my group asked me to stop due to being creeped out. I didnt. But they asked. It doesnt have to be long and certainly not continuous, but if you aren't going to roleplay a cackle, why play a witch in the first place?

To prove a point I once cackled for 20 minutes straight at a special. As a 15 point buy human expert I can pull it off. A magically powered witch with a 20 point buy better be able to.
I was finally asked to stop when we were about to start the next phase of the special.
Usually I only cackle briefly on my turn or when something my character is particularly pleased with happens.

In combat I haven't seen a witch roleplay it every single round. Each round only lasts 6 seconds so that's fair enough. Out of combat if someone wants to perpetually cackle I make them roleplay it (we are in real time after all). It's never lasted more than 5 minutes and they've never done it more than a couple of times.

I have yet to do this, but when I next play a witch, I'm going to have a MP3 on my phone prepped up of Kefka's infamous laugh. That way, I'll be like, "Standard action, I use my misfortune hex, move action, I keep my evil eye hex going by..."(pressed the play button, looks around at the group to see who gets it and who is facepalming).
I think after three rounds it'll be annoying, but think of how great it'll be the first time I spring it!
Really, any time you can reference Kefka, ethics obligates that you should.

OT but i used to bust out Kaeru's (Frog from Chrono Tigger's) theme when my Bard inspired courage. i'm actually severely disappointed every time a bard inspires courage and this music doesn't play.

Nothing wrong with that. I recently played Jerry Lee Lewis just to highlight that I was casting an empowered fireball.

In my Reign of Winter game the paladin and barbarian were both Ulfen. The bard would start off combats with "We come from the land of the ice and snow"

As a DM, I would not force any Witch to RP their Cackle spam.
As a player, I would not accept a DM's demand to RP my Cackle spam.
Why? Because I have asthma, and I would rather not play with my lung capacity.
If my asthma were that bad, and I had a GM make a demand like that, I'd give him a performance with less emotion than that one chick in Forgetting Sarah Marshall did with her sexual reactions.
Ha. Ha. Ha.

I giggle when my witch cackles, unless I'm using the quickened cackle ability from her cackling hag's blouse. In that case, I state it out. "Kory flies to within 30 feet, Big Bad X can make a will save DC 20 against Evil Eye, then she used her quickened cackle."
Our druid's player has a fun app on his tablet for weird sounds that he mostly uses as a soundtrack for my witch. This began when he got the Benny Hill Theme when my witch confused a large group of mounted targets.
I should ask him to get a cackle to play for me.
