I think that it's entirely possible to trigger an event that adds or changes a flag:
When a character attacks a non-hostile character, add the "Accidental? Attacker" relationship.
What a character attacks a character with whom he has the "Accidental? Attacker" relationship, replace it with the "Attacker" relationship and apply alignment/reputation penalties.

i faintly remember that there was also talk of an "involved(?)" flag? for characters in a fight?
If i remember that correctly, then, even if A´s attacker flag expires the two would still count as in the same fight.
-i would presume that no attacker flag is given out to characters in a running fight(new party gets involved - different story)
oonly question that would leave me with is, if A waits out the 30 seconds after his first attack, and then manages to kill D, would he still lose Rep?

They dropped the Involved Flag.
... the nuanced hostility means we no longer needed the involved flag.

There was discussion about that earlier in this thread.
As I said then, my assumption is that A would not lose Reputation if they waited the 30 seconds for the Attacker Flag to go away.

But that also implies that B kept attacking after the 30 secs, and therefor gained the attacker flag himself (and the rep hit to go with it.) then A could fight back without any penalties.
I could see some sort of a contest or duel of sorts LOL. Like 2 drunks in a bar trade punches till someone falls. a hits b 1 time, that starts a 30 sec timer for B to try to kill a rep free. If he fails he hits 1 time at the 31 sec mark flagging himself hostile to A, who has 30 secs to try to kill B. back and forth till someone wins, and the winner losses nothing LOL. Can't hit the 2nd time after the initial "tag" or you gain attacker and lose rep.

Let me clarify...."Attacker flag" = hostile state where you can be attacked by the person you hit first without rep loss to that person.
If I punch Nihimon, I gain attacker flag (Hostile state) and he can attack me (retaliation) rep free for 30 secs. If I hit him a 2nd time in those 30 secs, I take the rep loss and he is free to kill me for how ever long the flag lasts, 5 mins or whatever.
