There is no such thing, and were someone to create one, it would not be a valid chronicle.

Slight clarification on that - A chronicle can be a PDF form if it is the chronicle directly out of the scenario's PDF, and only the fields you would normally fill in by hand are form fields, and the form fields can be flattened and made non-editable after filled out by the GM.
In other words - you can add form fields to XP gained, GP gained, PP gained, Player Name, PC Name, Faction, that kind of stuff - and then after the GM fills it out they have to "flatten" and lock the PDF so it is no longer editable, before it is sent to the player.
What has been banned is people creating custom chronicle sheets that are not the chronicles directly from the scenario as it is in the PDF, or chronicles that have boons and things added to them, et cetera, that are not originally on the sheet. (One Example: You cannot make a totally blank chronicle template from season 5 and then copy the text from older scenarios to it just so you have Day Job and that kind of stuff on it - which has been a common practice.)
That said - on the original question, PFS leadership has indicated that there will eventually be Form Fill-able Chronicle Sheets made available.

There's no such thing as a properly locked PDF that a player can't unlock anyway. The scenario itself is locked, and that's where the chronicle sheet comes from to begin editing in the first place.
I agree with not modifying chronicles in the way that's been brought up, especially the craziness about people adding boons or even changing the format of chronicles to look like season 5, but it's always going to be up to the honour system for players to not falsify chronicles (online and unlocked, online and locked, or written).
