Does perception suffer penalty for long range?

Rules Questions

As the title says, does a PC or NPC take a penalty for trying to spot something at long range, such as 100 ft plus? The page for perception doesn't mention it but it seems that it would make sense.

Grand Lodge

Distance to the source, object, or creature +1/10 feet

It's in the chart.

As Cold Napalm said, its on the chart. Its not actually a penalty on the Perception check, it just raises the check DC. +1 to the check DC for every 10 feet of distance.

How does that work out with stealth? Usually a NPC only gets one roll to detect someone stealthed. Would they make this roll with the added DC or not?

Wouldn't make sense if it didn't.

In that case, let's say that I roll a 25 on stealth and they roll a 28 on their Perception. At 40 feet away, they wouldn't be able to see me but at only 20 feet away they could see me. Is that how it should be played out?

I quite often use the distance modifier as effectively a Perception range threshold: subtract the Stealth roll from the Perception roll. Multiply the result by 10 feet (if positive) to get the range at which the perceiving character notices the stealthy one - as long as the stealthy character stays outside that range they are hidden (with all of the caveats about stealth requiring cover or concealment to function).
