If I read it right, it works like this:
In hybrid or animal forms, you use the better scrore from base creature (i.e. humanoid) and base animal.
The base creature's scores are determined as usual (i.e. you use your purchase or point buy or roll 3d6 or 4d6, or use a standard or elite array, whatever you'd do for that character if he wasn't a lycanthrope.)
The base animal's scores are the very scores that are listed in the monster entry (For example, a wolf gets Str 13, Dex 15, Con 15, Int 2, Wis 12, Cha 6).
So in humanoid form, you just use the old stats. In the other forms, you use the better stats.
In addition, on top of all that, you add +2 Wis and -2 Cha to everything, and +2 Str +2 Con to hybrid and animal (on top of the wis and cha adjustment).
Example: Let's say that Valeros (as he is found at the back of "Souls for Smuggler's Shiv") is bitten by a werewolverine and contracts the therianthropy curse (yeah, I can be pedantic at times. And if it's not a wolf form, it's not lycanthropy!)
Valeros has Str 16, Dex 15, Con 12, Int 13, Wis 8, Cha 10
The wolverine has Str 15, Dex 15, Con 15, Int 2, Wis 12, Cha 10.
So Wervaleros in his human form will only get the general bonuses, (+2 wis, -2 Cha), resulting in Str 16, Dex 15, Con 12, Int 13, Wis 10, Cha 8.
The other forms get the better stats of each.
That is Str 16, Dex 15, Con 15, Int 13, Wis 12, Cha 10. You also apply the +2 Str, +2 Con, +2 Wis, -2 Cha, for a final array of Str 18, Dex 15, Con 17, Int 13, Wis 14, Cha 8.
