There was a d20 release from Blackdirge called "Lesser Half-Dragons" that had the half-dragon turtle, half-pseudodragon, and half-wyvern, but even that PDF didn't have a half-faerie dragon. It must have gotten pulled from drivethrurpg at the end of the d20 license and never reposted as OGL, I'm sorry to say.

Alright here's my take on it. Most of what I'm using here is from the Draconomicon version of the faerie dragon, except that I don't believe they should get bigger with age, so I consider them to always be tiny:
Half-Faerie Dragon is an inherited template that can be applied to any living, corporeal creature (not of the living construct type). A half-faerie dragon uses all of the base creature's statistics and special abilities except as noted here:
Size and type: The creature's type changes to "Dragon". A faerie dragon is a tiny creature, and its size is reflected in the size of its offspring. Lower the base creature's size by one category, unless the base creature is Tiny or smaller. For example, a half-faerie dragon/human is Small instead of Medium. This gives the half-faerie dragon a +1 size bonus to AC and attack, and a -4 on grapple checks, when compared to the base creature.
Hit Dice: Do not change the base creature's racial or class hit dice.
Speed: A half-faerie dragon has fairy-looking wings which give the creature a fly speed of 60 feet (good). If the base creature was Medium or Small, reduce the base creature's land speed by 10ft.
Armor Class: Increase the creature's natural armor by +2.
Attack: A half-faerie dragon has two claw attacks (considered a primary attack) and a bite attack (considered a secondary attack). A half-faerie dragon fighting without weapons uses both claws and its bite attack when making a full attack. The damage caused by these attacks is based on the size of the creature after applying the template, and can be found listed under the Half-Dragon template in the Monster Manual. If the base creature already has a bite attack or claw attacks, the damage stays the same as the base creature's (Note: since normally you would raise the damage in this case by one die, it is offset by the smaller size).
Special Attack: A half-faerie dragon retains any special attacks of the base creature and gains a breath weapon which manifests as a 20-foot cone which causes euphoria in the target. Any creature within the area of effect must succeed on a Will save or be dazed for 1d6 rounds. The save DC is 10 + the half-faerie dragon's Charisma modifier. This can be improved by taking the Ability Focus (breath weapon) feat, which raises the DC by 2.
Special Qualities: A half-faerie dragon is immune to sleep and paralysis effects, and has darkvision 60ft and low-light vision. It does not gain any type-based immunities.
Abilities: Modify the base creature's ability scores as follows: -4 Str, +8 Dex, -2 Con, +4 Int, +2 Wis, +4 Cha.
Skills: Racial skills are the same as half-dragon template (didn't feel like retyping all of that). A half-faerie dragon has a +4 racial bonus on swim checks.
Feats: A half-faerie dragon gains Weapon Finesse as a bonus feat, and it may apply this feat to its bite and claw attacks in addition to any weapons that qualify.
Challenge Rating: Same as base creature +2.
Alignment: CG or CN.
Level Adjustment: +2

Well, by your reference to a Grapple bonus I assume you're running under 3.5, but there is the Pathfinder version of the Faerie Dragon is in the Bonus Bestiary or else on d20pfsrd. You might only have a -2 Str penalty and a +6 Dex bonus, but otherwise it looks ok.

Does Pathfinder not include the grapple reduction for small size anymore? I didn't realize that one.
Yeah, I suppose the +8 Dex looks high, but you get a +8 Str with a Half-dragon, so I thought I'd keep it similar. Then again, I'm doing one less LA, so you're probably right, +6 Str.
I didn't like the Bonus Bestiary version because the breath cone is only 5ft (totally not useful for a medium character!!) so I stuck with 3.5-style for the most part.

Well, there is no more "Grapple" in Pathfinder. It's been replaced by the Combat Maneuver Bonus and Combat maneuver Defense. There is a size modifier to CMB and CMD equal to the inverse of the size AC modifier for each category.

I'm currently playing a half-faerie dragon in a 3.x campaign one of my friends just started up. No level adjustment, +1 nat armor, size goes down by one size class, some of the dragon racial abilities, and the euphoric gas cone as a breath weapon. And a fly speed.
To tell the truth, I thought he was absolutely nuts for letting me have the fly speed with a +0 LA template. But also I really didn't much care what he let me have as racial abilities, since I just wanted the cosmetic appearance/flavor of it all to play into the character concept.
