The biggest issues I'm having as a Kingmaker DM:
The kingdom rules state that the leaders need to spend 1 week per month doing downtime kingdom duties. The rules also say that a kingdom turn is 1 month.
Do players need to take a month of downtime to take a kingdom turn?
Or can they spend their week doing duties, go back to adventuring, and then a turn triggers at the end of the month?
Do kingdom turns trigger monthly automatically, or only when PCs want to do them?
Do they need to do the week of duties EVERY month, or only months that they want to take a kingdom turn?

The kingdom turn occurs every month regardless of player actions. The players ideally spend 1 week of each month attending to their duties. If any character that has a kingdom role is not present for that week, their position is considered empty. So if the players skip a month, the kingdom will take lots of penalties on the rolls that have to be made each turn - mainly stability and taxes, and possibly events.

Do the PCs need to be in the capital/kingdom hex to do their duties?
Do they need to be IN the kingdom at the end of the month for the kingdom turn?

By the rules introduced in Kingmaker, they have to be in the kingdom for at least a week out of each month. Not sure about changes in other kingdom-building supplements since then.
Some have suggested requiring them to be at the capital for a week. My campaign requires being at one of their cities for a week. But those are admittedly both houserules.

The kingdom turn happens overlapping each month, rather than a specific time each month. That being said, it simplifies matters to decide a particular time each month (such as end of month) to "take care of business", and do the rolls then. Even moreso if you have one of the above requirements about being in a city or capital.
