Given that you're required to provide "your villain's motivation(s) and goal(s)" and "schemes and plot and/or adventure hook(s) for your villain," I'd let the ands and ors tell you what to do with the headers.

This is not quite a formatting issue but it's pretty close. My villain would be most elegant if I use a custom template, and I could include this information in the stat block but obviously I can't this round. So, my current plan is to include the name of the template and make sure it's descriptive enough that everyone knows what's going on. A similar example might be something like: "Ghazghkull Mag Uruk Thraka, Spore-tainted Orc Barbarian 10 Fighter 4". So, does that work?

Vitallus, half-chogroth orc barbarian 6
I have no idea what a "half-chogroth" and it might as well say "floopadoopa." Spore-touched, however, I have SOME idea of what thats might be....

Multiclass villains are normally separated by a slash "/"...so it would be:
Vitallus, half-chogroth orc barbarian 6/fighter 2
You can see examples of this throughout Pathfinder products.
