I found a similar thread in the STAP forum but couldn't find one here. I'm looking for stories of great heroics from your PCs!
Mine from last week.
Character: Feravel, Duskblade 9
Adventure: Red hand of Doom
Situation: Nasty fiendish Behir attack
Having beaten off wave after wave of hobgoblins at the city, we got attacked by a big bad behir. Since the paladin had almost been pulverized by a dragon it was Feravel's turn to lead the charge. He used his most treasured possession - Stone Salve, and charged the beast. Unfortunately it was ready for him and swallowed him whole.
Imagine it's surprise when it got hammered with a Vampiric Touch and two Scorching Rays from inside it's stomach!
When he cut himself from its corpse no-one would believe that was his plan all along :)

Character: Robillard Laibach, Paladin 4/Wizard 4 of Mystra
Adventure: Legacy of Flame (Homebrew FR Campaign)
Situation: Priestess of Auril and a potent artifact.
A human cleric 9 of Auril was trying to seal off a portal to the Elemental Plane of Fire deep within the volcano near the city of Neverwinter by using a powerful artifact known as the Chillstone, a crystal imbued with the power of winter.
When the heroes arrived (it was only a 2 player game), they found the portal rapidly shrinking and an elder fire elemental attempting to squeeze through the gate to stop the priestess. They immediately sprung into action, attacking the cleric and diverting her attention away from the ritual. This allowed the elder fire elemental to eventually escape into the material plane (3 rounds later). The party was pretty badly thrashed after a couple of devastating frostfall spells (as flame strike, but cold damage instead) and the priestess kept healing herself, hoping to win by attrition. When the elder fire elemental emerged, the priestess hurled the Chillstone into the elemental, paralyzing it and rapidly draining it's life force. The priestess then abandoned the Chillstone and fled, leaving only Robillard standing (the other party member had been frozen solid by one of the other powers of the Chillstone). Instead of pursuing the priestess, Robillard ran to the elemental's side, grabbing ahold of the Chillstone and removing it from the elemental's chest which froze his own arm solid in the process. Robillard succeeded on the save against being frozen in place and quickly moved to the yawning portal in the center of the room, walking into the Elemental Plane of Fire where he shattered his own arm against a red-hot boulder to prevent the freezing from spreading to the rest of him and then kicked the Chillstone into one of the magma pits, destroying the artifact for good. The character went through another 3 levels before a regenerate spell was bestowed upon him to replace the limb, along with a holy avenger as a reward for his exemplary devotion to the church.
The elder elemental gave Robillard a tiny glowing ember in return for saving his life and destroying the artifact which he would later use (nearly a year later) to summon the elemental into battle against a specially home-brewed Ice Balor.

Character Name: Jamaicanmetan
Class: Barbarian 6
The party was exploring some ice caves in a frostfell setting, when they ran across a mad frost giant (no axe or armor). Giant charges, hits Jamaicanmetan for minimum damage with a slam (11). Jamaicanmetan goes next, and scores a double critical with a tigerskull club (X4). He jumped up, stuck the teeth into the giants eye sockets, and then rips off its face for 104 damage. The giant stumbles around blindly for all of three seconds before Jamaicanmetan backhands it with his offhand warhammer, killing it.

this one is somewhere between this thread and the DM kill board, but the PC didnt quite die.
Character: Thorn (Ranger Prick)
Class: Ranger 6
Further in in the ice caves, the party kills some degenerate cave-dwellers, and thorn and the ninja investigate a room sealed with a ice block. Inside they find a half-skinned dire bear hanging by its feet from the celing. Going to investigate, thorn discovers that the bear is not quite dead, and the bear grapples him! So the Ninja misses, the bear attacks and brings thorn down to 1 hp. Thorn draws his dagger and stabs it, the ninja sneak attacks it, and thorn is about to die when the bear rolls a natural 1 and drops him. Thorn, lying prone, quickdraws his greatsword and kills the bear with a critical. (it had very little health, on account of being both flayed and frozen.)

Character: Viscount Malcolm Evander Trinius Greenwood III (CG / CN male human fighter 2)
Campaign: Age of Worms (The Whispering Cairn), Greyhawk
When the attacking Wind Warriors cornered the party, Malcolm stood his ground, yelling back towards the other PCs to run. However, Girak (male human warmage 2) and Wic (male human sorcerer 2) had the same idea. Only Caraen (female half-elf rogue 2) escaped with her life.
Doomed and foolhardy, but valiant nonetheless.

Character: Sir Edmond Violet (LN / LG male human cleric (Wee Jas) 2 / binder 2)
Campaign: Age of Worms (Attack on Blackwall Keep)
1. Edmond rear-guards the party while they retreat to the inside of the keep. He remains outside the open gates, slaying two lizardfolk with his axe while the other PCs (a rogue and a sorcerer) went to the battlements to rain magic missiles and crossbow bolts down on the lizardfolk.
2. In the lair of the harpies, Edmond climbed a tree, leapt onto the back of a harpy (natural 20 jump check), and dragged it down, allowing the party to slay the beast with their melee weapons.

Character: Nicarus Grey (Rogue 5/ Monk 2)
Campaign: Shackled City
Chapter: Drakthar's Way
The Vampire Bugbear had charmed our pt's Brb/Fgtr with her low will sv. and told her to protect him. My bright idea: I tumbled through her threat range (reach weapon weilder) and initiated a grapple. Though risky, the gods smiled on me that day. I subdual-ed her to unconsciousness, tied her up and shackled her then went for Drakthar. I used my decanter of endless water to make a stream of moving water that trapped him in a corner where our party was able to defaet the beast and drive a wooden stake through his heart. From that day forward my pack contained a small mirror, wooden stakes, a mallet, holy water, strand of garlic, and a sharp kukri for head removals. And grapple has been permanently earmarked in my PHB.

The setup: Deep Lowerdark (FR for this one), a deep, wide chasm with a thick stone bridge crossing it.
The battle: A Deep Dragon (Ancient with some buffing)
The Character: Rularis Kelgarde (male Human: Warmage 6/Rogue3/Daggerspell Mage 8)
Wings of Flying, +3 Saber of Sharpness, Belt of Giant Strength, far far too sure of himself.
After a protracted battle with a great deep dragon, much of the party was wounded (including Rularis, who had 9hp), after the party Wizard hit the dragon with a nasty horrid wilting, leaving it badly wounded, the dragon wingovered to attempt to retreat. Everyone else gave up, not so Rularis.
Flying Charge to the Dragon. He missed. One grapple check followed by a diving Crush attack by the dragon, and little gooey Rularis-bits were smeared all over the wall of the chasm and the dragons claws. Stupid, yes...but it was heroic. (Actually I think he just wanted its treasure..greed how I love thee as a motivator).
