There are several threads here in the SCAP forum about this. A few may have been archived by now; try a search if the thread titles aren't helpful.
The most common conversions seem to be:
St. Cuthbert -> Helm
Wee Jas -> Kelemvor
Kord -> Tempus
Pelor -> Lathander
Striders -> Harpers
Ebon Triad -> Bane, Bhaal, Myrkul
Cauldron in or near Chult.
My own basic setup was a bit different:
St. Cuthbert -> Torm
Wee Jas -> Oghma
Kord -> Kelemvor
Cauldron placed in mountains northeast of Neverwinter and Mount Hotenow.
(A few minor liberties with local geography and the Neverwinter Woods were also taken.)

dinosaurs -> dire boars
The nature of some of the changes meant the focus of some of the temples changed from what was presented in the adventure, but this had to do more with my group than anything else. However, I did think it was a bit old and tired for a church associated with death to turn out to be harboring some of the bad guys, so I went for something a bit different but still plausible. Since Age of Worms makes reference to a few things from SCAP, you may want to check the Age of Worms Overload and other conversion notes for a few of those details, but that's really not needed. I don't have the HC, just the mags; there may be more notes there.

I haven't run SCAP in the FR, but there is an interesting alternative that one could consider: Tashluta.
It is an existing city east of the Chultan peninsula, close to the Black Jungles. You can see a detailed map of the area there: http://www.wizards.com/dnd/images/mapofweek/Dec2005/02_SerpentKingdoms_72_p pi_cv2891.jpg
There is an entry on this city in "Serpent Kingdoms" and it is presented as a city laying in a dormant volcano.
So you already have two of the main features: volcano + jungles.
The line up of gods is interesting in that there is a quite widespread, official cult of Talona that poses as saviors against the Yuan-Ti threat, but are actually evil.
If you were placing SCAP in that city, you could replace Wee-Jas with this specific cult of Talona. You lose the focus on magic, but you retain the darker aspect of the deity.
The characters would then get a chance to unmask the Talonite corruption and show that not only they work to spread corruption, but they are also alied with some dark cult that tries to revive and fusion the Dead Three!
I would try it that way, had I the time to run SCAP.

I did things a little differently with my dieties as well. The main roll that Kord seems to play in the campaign is the creation and running of games in the flood festival. To that end it actually makes more sense to me to have Lathander be the replacement for Kord.
The shrine of Pelor needs to be small and paranoid and pissed off when the temple next door gets built so high it can't see the sky. Since Lathander was already used I think that the moon goddess fits this roll better.

I placed Cauldron in the south part Tashalar, in the mountains right next to the Black Jungle. St Cuthbert became Tyr (justice, retribution), Pelor --> Lathander, Kord --> Tempus and Wee Jas --> Malar.
The reason I went for Malar is that in my opinion, it is the evil aspect of the clergy that plays a role, not the portfolio of death and magic. Thus Kelemvor doesnt fit, he's always been a good guy, even if his official alignment is lawful neutral. I can't imagine a priest of Kelemvor sending assassins after the heroes of the city. Plus Malar fits the jungle theme perfectly.
The Ebon Triad will be Bane, Bhaal and Myrkul.

Just a thought: what about replacing the Spell Weavers by the Sarrukh?
If you set Cauldron anywhere on the Chultan peninsula, it would fit. They are quite similar in function as well: mithycal beings that are very few, masterminds, mastery of magic, etc.
The tablets and relics would be linked to the fallen empire of Serpentes and the Sarrukh and (looking at the AoW AP), it would be a Sarruhk Lich Necromancer that would have tipped off Kyuss on how to create über-undead.
The Sarrukh is much more FR-like than the Spell Weaver, IMHO.

I used Tashalar as well and placed Cauldron in the western part of the Hazurk mountains, west of Tashan gap. More or less I used the common conversions Ultrazen gave above, except Embril Aloustinai and Ike Iverson are Oghma's priests turned evil by their researches for Fetor Abradius and the Cagewrights on Adimarchus and the spell weavers. IMC, Ebon Triad are Bane, Velsharoon and Shar. It's not as logical as the official conversion but I wanted to use Shadow Weave heavily. I also used the yuan-ti background from "Serpent Kingdoms" as the most active slave buyers in the region.

I've used Azuth instead of Wee Jass in cauldron. Bocklin, thats a great idea! It would explain the ties to the kopru in flood season as well. Nabrothon? in the demonskar could be allied with the khasta? in their hunt for the Sarrukh,which could be one of the main reasons he was there at all.

I am also setting my version of the SCAP in the Tashalar, though I'm placing Cauldron in the dead center of the Tashan Gap. From reading the Serpent Kingdoms book, it appears as if there are low mountains within the Gap itself - perfect for my Cauldron. I'm keeping the Cauldron Environs map as is. Tashluta will replace Sasserine.
Other changes:
St Cuthbert = Helm
Wee Jas = Kelemvor (with Embril and her cronies being corrupted Cyricists masquerading as Kelemvorites)
Pelor = Selune
Kord = Tempus
Spellweavers = Sarrukh
Vortimax Weer = Thayan wizard w/ goal of establishing a Thayan Enclave in Cauldron
Since my game doesn't start for another month, I'll definitely add more.
