Well now I have a player asking me to have his goblin rogue character carry a sack of tied up puppies around so he can hide under tables, bite the puppies for the morale buff and then sneak attack bite people. I'd be less amused if that didn't sound exactly like a plan a goblin would come up with if you told him biting puppies made him better at biting other stuff.

I don't know how that would even work, aside from just intentionally doing something to provoke. Realistically it would be more like just holding action until after the enemy attacks you. I assume this is an issue because there's some reason you want to trigger an AoO, so as a DM I'd probably assess what trick it was you were trying to pull, and allow or disallow it depending on what it was. But officially, no I don't think there's anything in the rules that says you can do that, because an AoO is sort of assumed to always be something you don't want to have happen.

Assuming they got out if it as soon as possible I don't think it'd be an issue. At least I wouldn't penalize them if I was a DM. If you had a monk or something with a vow not to drink alcohol, I wouldn't penalize them just because someone with a syringe arrow shot them with a shot of jaeger from across the street.

As a player, this was the first time I ever actually had a character get killed.

Catalyst:Trio of Zombie Dire Tigers aka the Blender Bengals
Gory Details: The rest of the party being slightly roughed up, Eshanya decided to be the first one into the next room, taking it a few steps ahead so she could warn anyone before anything happened. Never got the chance. Had just enough time to see them leaping at her before rolling poorly for initiative. Three Tigers, Three Pounces, Three Crits, one absolutely pureed catfolk. She got raised eventually by the cleric, but she opted to retire from adventuring after that little misadventure. It wasn't quite so fun anymore. Playing an Investigator now, which has been fun.

I'm really, really new to GMing PFS, but it would be nice if I could let my friends legally replay for no credit. I have a pretty small group of players, so if one of them misses a game, we can't field enough folks to put together a legal table for the person who missed out to play. I've ended up just running a them again without it being an official PFS game if the player really wanted to try it, but I feel bad those players won't be able to get credit for that scenario.

I get the spoilers issue, but frankly, I'd rather let people play and deal personally with disruptive players, than have someone miss out on the off chance someone /might/ spoil things.
