As noted above the poltergeist's fear effect has the incapacitation trait bumping the failure to a success.
I'm not familiar with that, I'll have to do some looking around, I haven't dealt with incapacitation rules. Also, Meat Wagon should have repaired 30hp and would be down to 32 now after the fire.
Stitch ◆ draws out his pistol and gives the ◆ orders "Full firing squad at the last rat!" He aims and pulls the ◆ trigger, burying a steel ball into the wall behind it.

(Not sure how far the fetid flames aura extends. Am I in it? No flat check needed as I can see through smoke.)

The final rat falls to the combined assault of the pathfinders.
Tonfer puts the finishing touches on the weapon while the group extinguishes the burning Quickblade and Meat Wagon. The axe glows faintly and emits a small plume of smoke. The smith carefully wraps the axe in a heavy leather tarp. "Well that could have gone better. It looks like there's a slight quirk in the axe causing that smoke. Maybe the rats had something to do with that. Seems harmless enough though." He looks to Vardarel and Khiari. "Maybe with a little more practice you'll be able to do better next time. Your goblin friend was a big help though. Stitch is it? I've got a big order to fill for a merchant from out of town. Maybe after you finish up your business with Viltydus you could stop by and give me a hand." He digs around the wreckage of the forge until he finds a small chest. He opens it and hands the group a sack of coins. "Most of you may not be blacksmiths but you sure know how to kill monsters. Thanks for your help. Now if you'll excuse me I have some tidying up to do here. I'm sure Viltydus is eager to hear from you."
We can hand-wave any healing. You have plenty of time to treat wounds.

Fetid Fumes Aura 5 ft A creature that enters the aura or begins its turn there must succeed at a DC 22 Fortitude save. Everything within the aura, including the rat, is concealed by smoke.
I could have been a little clearer. Clover was not in the smoke.

"Yeah, okay, that makes sense. I did ask for fire, but not for smoke. Maybe I angered someone." Khiari apologizes. "Excuse me." Then she begins coughing up as much smoke as she can.
Bella is much more polite about it, but eventually she draws a handkerchief from one of her more annoying suitors and makes a mess of it.
"Sorry about the rat guts!" Khiari grabs another one of Bella's suitor's handkerchiefs and wipes the gunk off her horns. Bella accepts it without hesitation. "I bet Viltydus wants an update... Busy little fey."

Stitch's attention is quickly gained by both the compliment and offense, he whispers in response [smaller]"Thanks, but despite outward appearances I'm actually a Dwarf, this goblin appearance was just a horrible accident." He grimaces at the never ending reminders of his mistake.
"I'd be happy to come back and help with your order." He accepts, knowing full well its always good to study techniques from any and all masters.

While the others work on getting situated, Stitch fixed up Meat Wagon and resets his instability. "Any need a transfusion? I got a spare bag of blood..." He motions to an old waterskin hanging off the side of Meat Wagon.

Quickblade emerges a bit awed by what he just witnessed. "My, the chronicles I read about this group, as told by Mr Thunderbird weren't exaggerating. Though I'm curious about a side note added by Bun Helsing that Roark doesn't write his own stories?"

As the sun begins to descend beyond the horizon Pathfinders return to Viltydus's stump. The fey nods as the group recounts the events at the forge. "So the weapon now constantly emits smoke you say? Interesting." They shrug. "I have no more tasks for you today. Come back at first light and we shall continue."
During dinner, Imara approaches the Pathfinders. "I went to talk to Viltydus after supper. They weren't at their stump so I called out. They came out of the forest and told me to come get you all. They said it was urgent. They'll meet you on the path leading into Forest of Trials by their stump."
Following the path for about a mile the Pathfinders come across Viltydus standing amidst broken branches, scuffed tree trunks, and spatters of dried blood indicative of a struggle that started here and progressed further into the wood.
Viltydus lets out a deep, slow sigh. “My life’s purpose is to prepare heroes to enter the forest, to guide them to victory. Sometimes, though, the forest proves too dangerous. For every warrior who finds their way to Valenhall, a dozen others fall before they reach their goal. I sense one has fallen this night. Pathfinders, your task is to recover the one whom the forest has felled. Bring them home alive if they can be saved. Return their remains for final services if they cannot.”
The path is riddled with signs of conflict, so tracking should be simple even in the dead of night. Finding the fallen adventurer requires one successful Perception or Survival check, or one successful Scouting Lore check. One character can make the check and others can aid.
One hero point to Vardarel.

"That is unfortunate, Viltydus. We will recover the survivor with all due haste." Bella scans the sky. "How much light do we have? My sister can see well in low light situations, but I am not as gifted."
Perception +10 is my best skill. I can lead or aid but I suspect people have better skill check bonuses.
