Everyone can post their actions whenever, and we can change around/retcon as needs be.
Mael, with a fearsome growl, hefts his club and charges straight for Eudomia. He's now located at G6, and has a -2 AC from charging.
Charge Attack: 1d20 + 3 ⇒ (9) + 3 = 12
Damage: 1d6 + 1 ⇒ (4) + 1 = 5
His downward swing connects with Eudomia's foot, and she swears at the burst of pain.
On her turn, Eudomia retreats backward (to E4), drawing back her bow and firing point-blank at Mael.

However, her injury throws her off balance, and the arrow whizzes past Mael, careening into the ground.

Firstly, Mael doesn't make an AoO. Mael's flat-footed during Vhiloe's turn, as he hasn't acted yet in the combat.
As Vhiloe has no threatened area, he can't get an AoO for an enemy moving out of his threatened area, or so I believe. However, he could still harry someone during spellcasting, or another opportunity where one might get an AoO.
Mael shrugs off the sharp beak of the bird, concentrating his attacks on Eudomia.

Hefting the club in his hands, Hathin charges forward at Vasile. The club is gripped in two hands and brought in a brutal cross-wise strike at the girl...

You might want to change targets Vasile, If Hathin drops unconscious from Silyphs spell. Pakak acts after you so he makes a nice target ;).
Silyph is suprised by the swiftness and bravery of the tiny bird as she thinks to her self, "I should feed him nicely tonight." then in an instant she is further surprised by the swift reflexes and ridged determination of Mael.
She calls out to the her team-mates. "Split up it'll be harder for them to corner us if we're not together." She gets a wicked grin on her face as she and shouts "If this works don't worry about that stronger looking orc-kin, he'll be out cold for a while!" before she mutters the arcane words to of enchantment.
She then 5-foot steps to D5 and casts Sleep she'll choose the target once he spell comes into effect at the start of her turn.

Pakak, seeing that the battle is moving far beyond the pillar, moves up and closes with the nearest 'foe' I believe that's Eudomina, and if he can do it without setting fire to an ally, casts his burning hands.

Yeah, Hathin could have 5-foot stepped closer, and I'll assume he did because he made the attack roll assuming he was 30 feet away. So Vasile is knocked unconscious.
Pakak moves in between Mael and Eudomia, casting a spell and shooting a thin wave of flame from his fingertips.
Eudomia can't threaten with her bow, so Pakak doesn't need to make a concentration check.
Eudomia gets most of her body out of the way, though the line of fire catches her straight across her bicep. She lets out a gasp of pain as it strikes her.
I'll rule that her arrows don't catch fire, and it won't ignite your guy's spell books either. I've never liked a 1st level spell completely gimping characters in that way.
Mael stands dazed from Silyph's spell. However, he seems to be getting a hold on his senses. Ends on Silyph's turn, right after Mael.
Eudomia sidesteps away from Pakak (to C4) and attempts to cast a spell. However, as she begins to cast the spell, the air around her glows a soft purple, and her spell is transformed into a primal magic effect.
Percentile roll: 1d100 ⇒ 10
Two giant centipedes (size Medium), one green and the other blue, appear in the area (at D3 and C5). One attacks Pakak, while the other attacks Eudomia.
Attack vs. Eudomia: 1d20 + 2 ⇒ (2) + 2 = 4
Crit confirm: 1d20 + 2 ⇒ (20) + 2 = 22
Damage: 1d6 - 1 ⇒ (5) - 1 = 4

The centipede bites into Eudomia just as soon as it appears, and she looks close to falling. Pakak avoids the other centipede's bite.
New Map: Round 3, still need Silyph and Hathin's round 3 actions. I used beetles instead of centipedes because that's what I have, and I'm being lazy right now.

Silyph glances around seeing Vasile fall Eudomia taking a beating and 2 new giant bugs as foes and decides.... "there's no honor among....well....wizards...." hoping the chaotic environment hampers her foes movements as she runs as fast as she can to the east provided there's nothing in her way that blocks her path she'll stop at the first obstacle that blocks her path. She'll run 110 feet the east if that's possible. We run out of letters and get to Z at about 80 feet if we double up letters after that she'd be a (Ad,1) or 22 squares to the right.
She shoots a quick glance at Eudomia before calling out "Sorry dear, but I need a little space between me and these boys. It's not you it's me and I don't fancy their large sticks very much. Good luck!" before dashing off as quick as her feet will carry her.
She turns her head over her shoulder and shouts as she runs "Hey Mael, you know my little birdie is right you are just a big dumb meat head, how ever did you get admitted to this place. Betcha can't catch me!" a concealed grin forms on her face as she continues to run.
The only thing I see weird about the map is that Hathin would be at G4 rather than H5 if he did his 5-foot-step. And I believe you're still waiting on actions from both Hathin & Pakak rather than just Hathin. *tips hat*

Bereft of his weapon, though thankful that he still stands, Hathin growls as he moves towards Eudomia. Closing the distance he lashes out with a meaty fist...

Mael moves to finish Eudomia off with his club, charging (to D5) and swinging furiously.

He connects with her neck, and down she goes. The centipede that was attacking her then seems to lose interest, and begins attacking Mael (on Eudomia's initiative count).

Vhiloe looks astonished by the giant centipedes and thinks of all the centipedes he's eaten before realizing they can now eat him. He immediately breaks off his pestering attacks and does a full withdrawal towards the sky.

Totally messed up last time, meant to run 100ft rather than 110ft. Would you mind adjusting since it's a full-round action so cutting it short should be possible and putting Silyph at Ab1 rather than Ad1?
Silyph spins around after completing her dash across the arena she quickly spins around and continues her taunts. "Oh wow it took 3 big strong boys and a pair of giant centipedes to finish off one little wounded elvish girl? You three sure are brave ganging up on her like that. It's two down one to go, are you going to play in the dirt with sticks and bugs all day or are you going to stop being boys man up and duel?" and on cue she takes a single step up to Either (Aa,2) or (Ac, 2) and begins chanting in infernal as her hands drawing complex motions a small red pentagram glows hot red on her steel bracelet.
"Ego vocant ad materia regni .........."

The 100-foot move is fine instead. They'll probably not be hearing your taunts very well, as they're pretty far away, and in the middle of other things.
Finishing off Eudomia, Mael turns to find where Silyph went. Seeing her a ways off, and also seeing that she's casting something, Mael will start to move toward her, having to dodge around a large patch of ice and thus not being able to run. Double move, ending at O3. The centipede gets an AoO.
Centipede Attack: 1d20 + 2 ⇒ (2) + 2 = 4
For simplicity's sake, we'll say that all the terrain past the edge of the map is not difficult or hazardous.

As Hathin nears in the distance Silyph completes her spell her steel braclet glowing bright red for a in instant before fading. Her hood still over her head all that can be made out by poor Hathin at that distance is a crooked smile as Silyph draws her spell to a close. (Finished Casting Summon Monster 1 w/ Master of Pentacles for +2 to CL & Summoners charm boosting duration by 1 round)
An split second later a large black dog with mangy fur and crimson red eyes appears snarling and foaming at the mouth. (Fiendish Dog is summoned @(U,2)
Silyph tosses back her revealing a happy glow in her eyes that can be seen from the distance. "Sick 'em Fluffy!" she cries out gleefully.
Fluffy barks fiercely as he moves to (P,2) and jumps up his jaws ripping ferociously into Hathins neck.

She praises the pup, "awweeauuahhhwww that's a good girl, who's a good girl, who's a good girl, you are, you're a good girl." she coos Hathins blood still dripping from the pooches mouth pooling on the ground with the excess saliva.

With his round 5 action, Mael moves to O3, looking to close with Silyph quickly. He uses a withdraw action to avoid AoO's from one centipede, and the other missed. I already rolled the one earlier.
Even when the vicious dog bites into Hathin, no blood is spilled. Even though he's ripped open, and you can see the flesh underneath when you look close, the wound doesn't bleed, and quickly closes itself.
Mael's eyes open wide at the sight of the dog, though he says nothing.
Both centipedes now attack Pakak, as the others left him.
Attack 1: 1d20 + 2 ⇒ (4) + 2 = 6
Attack 2: 1d20 + 2 ⇒ (14) + 2 = 16
Damage: 1d6 - 1 ⇒ (6) - 1 = 5

That's it for me then, Pakak realizes, however 'fake' this maybe, it feels real and even if he takes out these two things, no doubt the dog and conjurer will be able to take him out in such a weakened state. It's all just a matter of who gets him first, and Pakak knows that. Then an idea hits him Maybe they just need a new target. Course, these could just turn into more centipedes...but that will at least make it quick

Totally forgot about my little birdie; Since Mael moved before Silyphs turn go ahead move her backwards to (Ad1,2) instead she'd want to keep space between herself and him (he acts before her) and wouldn't advance if there was somebody that can hurt her so close.
Round 5:
Vhiloe flies over head and gazes down on the situation. He lands on His mistresses shoulder off in the distance and swawks, "Didja see those giant bugs! Pwwweeeurrrghh" Vhiloe shudders as his feathers ruffle.
Sorry for the map confusion, my bad. Ready for round 6, make any AoOs from fluffy against Mael's movement as necessary a hit initiates a free trip attack.

Pakak is inside the primal magic zone, so his spell goes awry.
Pakak begins to cast, intent on distracting the centipedes. However, the purple haze that surrounded Eudomia during her spell now engulfs Pakak, and the magic energy is redirected into:

One of the centipedes curls up into a ball, stricken quite dead by a sudden rush of negative energy emanating from Pakak. All around the arena, a low, otherworldly wail is heard as the spell is cast.
Mael, with a look that's part grim determination and part grim satisfaction, attempts to dodge past the fiendish dog and charge Silyph.
AoO for the fiendish dog: 1d20 + 3 ⇒ (14) + 3 = 17
Damage: 1d6 + 3 ⇒ (2) + 3 = 5
Free trip attempt: 1d20 + 3 ⇒ (13) + 3 = 16
The dog bites Mael's leg, and pulls him to the floor.

I think initiative goes as follows:

I adjusted Silyph to stay out of Mael's charge radius at (Ad, 2). Only posted up her movement in round 5 without knowing if Mael was accompanying Hathin. Silyphs got a 7 STR and is a summoner so she's very much not interested in closing distance unless she's cornered or her summoned dog gets neutralized.
I believe damage taken by Mael is (4 from Eudomia, 2 from centipede, 2-non-lethal from Vhiloe, 5 from fluffy). Bringing him to 11 damage 2 non-lethal.
Don't think we'll need a map Silyph will stay where she is outside of Mael's range and move appropriately as he does until her dog is gone. So I'll post up actions for round 6 currently.
Round 6:
His mistress shooting him a cold glance Vhiloe flies forward in a mad charge. "Ahhh it's dumb dumb! Didn't Vhiloe teach you lesson lesson! with Beak Beak earlier! You no have giant bugs protect you Vhiloe peck your eyes out!"

Mael, sturggling with both the dog and the bird, shouts out, "Agggh! I yield! Call your damn animals off!" He looks pretty messed up, and you'd guess he can't last much longer against Fluffy.
There's still Pakak as well, though it seems he's given up on the proper fight as well and is only concerned with the centipede.
Pakak, just so you remember, you're staggered right now, so you can only take a single actions each round. I know you did that this turn, mostly just putting it down so I remember.

@Shane: Pretty sure the centipedes droped him below 0 and he used his orcish racial ability to cast daze as a standard action before dropping unconscious. Plus the negative energy from primal magic might have done him in if he was still up.
Round 6:
Fluffy
The black dog growls as Mael hits the floor a lust for violence in it's crimson eyes. He lashes out at the prone Mael his jaws ripping into Maels neck a lack infuriated and raging at not being able to taste the sweet rust flavored juice of spilled blood.

She readies an action to cast acid splash should Mael draw any closer.

Mael stops resisting, apparently either killed or at least unconscious from the vicious dog's attack. Fluffy continues to "play with" Mael's body until you call him off or he disappears.
Edit: I was misreading, he's at -1, so he's unconscious after killing one centipede.

Presuming there are no apparent threats...
Silyph cautiously approaches the bodies of both Hathin & Mael. She gives a cold untrusting glance at both of them and casts acid splash on both bodies to insure that they're not simply faking it before approaching her pup.
