Round 1
Despite having no targets, you have the jump, and can react or cast spells or move defensively. It is hard to tell what or how many something are under the floor trying to get out. You are sure it is more than one however.
Take your actions. If we don't hear from everyone after a certain time I can bot them.

Gunther the Grim |

Gunther drops into his defensive stance with an arrow readied, eyes peeled for enemies. The look on his face shows he clearly intends to pincushion anything that appears.
Gunther enters Ranged Defensive Focus (full-round action).

Qahnaarin |

Not liking the idea of facing an enemy he cannot see, Qahnaarin leaps onto the stone fireplace and keeps his spear handy, ready and waiting for anything to appear.
Can I get an AoO if something is moving up and into a threatened square? Seems it would be like standing.

Champawat |

With the hair on her body standing on end, Champawat moves back over to the stairwell and calls down "everyone okay down there? What's going on? What was that noise?" She reaffirms her grip on her fauchard to strengthen her resolve. The curiosity of the events unfolding below her starts to wear on her.

Round 1
Prar casts Mage Armor on himself and moves to the center of the room to be near allies.
Goivan slips into the rubble and hides in the shadows, near Qahnaarin to provide possible flanking.
Goivan Stealth: 1d20 + 11 ⇒ (19) + 11 = 30
The others prepare for ..something..
As the group listens to the noise of something forcing its way up through the floor, the flagstones buckle and pop from their mortar as five different creatures crash through the thin floor stones.
Putrid rotting corpses crawl from their entombment beneath the floors, their red eyes glowing malignantly. They step from their graves and stand, wielding wicked longswords. They gaze at the group and wail a gutteral screech as they sense your life forces.
Qahnaarin has reach and is able to take an AoO against foe #1. The numbers might be hard to see without zooming in.
Above in the tower, Champawat hears the commotion, noise, and unnatural screeching. Something definitely is not right downstairs.
After the AoO, we can do Round 2, everyone take your round 2 actions

Gunther the Grim |

At this level, Ranged Defensive Focus lets me take AoOs against anything that provokes at 10' and 15'. Does that mean I can take a free shot against 2, 4 and 5?

CaveToad |

At this level, Ranged Defensive Focus lets me take AoOs against anything that provokes at 10' and 15'. Does that mean I can take a free shot against 2, 4 and 5?
Yes, you get one against them all with combat reflexes. I meant to look that up but got sidetracked by meetings today. Shoot away!

Undead 6

Champawat |

Champawat moves towards the sounds of battle and surveys the situation. These...things, were threatening her family. She had to protect them. She runs straight at the nearest opponent and slides low trying to slip between the creature's legs before popping up and continuing on towards Prar and Gunther.

Unfortunately his arrows bounce harmlessly off of the undead's armor.
Prar channels holy power through his focus and bathes the area in positive energy, blasting the undead. (damage is 1 regardless of save, with a minimum of 1 I believe.)
Champawat races into the room, sliding under the undead, between its legs and popping up by the fireplace in a dashing show of agility.
Also botting Goivan.

Qahnaarin sweeps his spear about, against his foes. (I think you meant 1 and 2, as #3 is farther away)
He is unable to drop his first target, but the second one drops to the ground.

Botting Prar
Prar moves slightly to minimize attacks casts Shield of Faith on himself.
Cairn Wight AoO vs Prar AC 17: 1d20 + 6 ⇒ (8) + 6 = 14 - miss
Champawat slams her fauchard into one of the undead and it hisses as a huge rent is ripped in its armor and skin.
Qahnaarin is unable to follow up on his attack against the one he tripped. (completing round 2 action for it) It stands and closes the distance, and Qahnaarin drops him again. He tries the same for the undead Goivan is fighting, but it is nimble enough to not fall.
Goivan, yes 17 was hit flat footed AC
Goivan continues his holy flurry, smashing the undead again with his axe and white light flashes. Unable to bite through the creature's armor, he lowers he head and rams his thick horns into the wight, lifting it off the ground as white holy light sears the undead creature who shrieks in pain and smoke pours from its eye sockets as the dark energy flows from it. It's smoking corpse drops to the ground and its sword clatters on the floor as Goivan destroys the unholy creature.
Waiting on Gunther's actions, otherwise I will bot him too.

Gunther the Grim |

Gunther sets an eye on the wight threatening Prar, trying to size up the undead creature. He then manifests another arrow and takes a shot at it, hoping to lay it low.

Champawat brings her fauchard down hard on the one she is fighting and deals a significant amount of damage. The creature hisses a hollow echo of rage from its decrepit body.
Goivan's double axe attack against his foe nearly drops it. The twin slices of heavy axe blows open two wide gashes in the creature and it reals with the ferocity of the attack but doesn't go down quite yet.
