Lastwall is a particular 'knights & lords' staple. I'd figure they don't have a standing army per se- more of a collection of lords & bannermen.
A loose rule of thumb on rank structure:
Lords
non-landed nobles (aka, knights)
men-at-arms (professional 'commonfolk' soldiery)
conscripts & levies
Another thought:
Many (if not all) of Golarion's kingdoms are ripped from the pages of Earth's history, with varying degrees of transparancy.
If you're going to set a campaign in Lastwall, by all means read up on the Reconquista. Only in Golarion, it's the Europeans who are putting up the fatalistic, doomed resistance to the unstoppable tide...

There was a writeup on the city of Vigil in Cities of Golarion.
I don't have the book in front of me, but I remember something about a shield tattoo and a sword tattoo determining rank of citizenship. This may have only been applicable to the city. It's been a while since I've looked at the book. I think it was in a sidebar
There was also a little bit of the overall hierarchy being led by a 19?-year old paladin.

I believe the guy that pulled the castle out of the deck of many things to create castle everstand was called a sargeant. I also remember in cities of golarion there was a "parade of nations" that among other things honored a recently promoted brigadier general. I personally disagree with these choices, but that's what is there.
