Is the mithril cost added before or after the creature mod?

For a large creature, it lists x4 cost. For a breastplate, is this +16000 for the mithril or is the +4000 flat? There is no change in price for medium or small armor, so it is possible that there is not cost increase here as well

You take the total price of the armor as it would be for a Medium wear and THEN apply the multiplier for the cost. This is for the base armor before enchantments are applied.
