It seems that for some of the questions, the answer I give doesn't send me to the correct next place. For Puddles, I said I used Valeros, but I went to the custom character sheet. For Puddles, no matter which I selected for the hit points, it took me to the reached zero option, and for Haven, even though I said I didn't play it, it continued as if I had. I do not want to give improper info, but I also do want to provide feedback. Anyone else noticed this?

Yup, other people have been saying the same thing.
I'm not sure what to do about it right now (wait until the team is able to get it fixed maybe?) but at least you're not alone.

You can actually skip answering the zero hit point questions. I had that issue. I selected I never went to 0 and then it asked on the next two questions what happened to get me to 0. You can leave those blank and move on.
The only reason i hesitate on doing this for a survey. Is it is possible there are other questions that would be more relevant, and thus more useful to answer, if fixed. (Such as "since you did not reach 0 hp. Did you feel the encounter was too easy/hard/just right?" As balancing encounters to players goals is another important step imho.)

I thought I had just misclicked on the 0-HP question, so that's a relief of sorts. Since I couldn't go back, I just filled in the information for the one time we had a character go down during the adventure. Hopefully that doesn't throw things off too much.
Out of curiosity, are there plans to add comment boxes to the survey going forward? My fellow players and I had more specific thoughts we wished to share, but we had no way to do so within the designed instrument.
