Right now I have a party at L12 about to enter a complex full of fiendish giants in order to destroy an altar dedicated to Lamashtu.
The place is populated with various giants, but I would like to throw in a few other creatures to make it more interesting.
Right now I have:
* A lamia matriarch leader with sorcerer levels
* A whole lot of fiendish ogres - the front lines of these are Fighter 5, the lesser ones warrior 2 (the warrior 2 ones are more of a speedbump)
* Some ghosts controlled by the Lamia through an artifact
* An allied tribe of Shagra (hairy ogres) many of whom are werewolves
* An allied tribe of hill giants, many of whom are ghouls
* The Nabasu demon that turned those hill giants into ghouls - he now has a pile of growth points (he is an object of worship for the hill giants as an emissary of Kabriri)
What I figure I need are some guard animal type creatures and maybe some summoned creatures. I don't want to use a Glabrezu or Nalfeshnee as these are coming up later in the game.
I'd also like to throw some dangerous small creatures at them. Most of what they have fought lately has been large or bigger.
Any suggestions?

L12 makes just about any animal-type monsters little more than speedbumps. You could maybe use them as mooks, but apply the advanced and giant templates liberally.
Winter wolves or Nessian hellhounds might work as the giants' dog-packs.
Swarms can be dangerous if you get the right ones.
The aurumvorax (think admantine wolverine on steroids) is small but very dangerous.
Vrocks are likely as summoned creatures; they or hezrou would work nicely with the fiendish theme of your castle.

I like the swarm idea; the party hasn't faced them in a long time. Any suggestions? Are there any swarms you might suggest that have fire/electric resistance? Or an ability to close quickly with the party? My party has a blaster sorcerer that throws a lot of fire and lightning. I don't mind covering the whole map with little bugs.
The Nessian hellhounds would work well but I might re-skin them as CE abyssal creatures rather than LE hellspawn.
I like the Aurumvorax (what is the plural of that? Aurumvoraxi?) also, since they are small and still pretty dangerous.
The vrocks are the right CR but there's a point in the adventure that highlights a rivalry between Pazuzu and Lamashtu and the Vrocks are supposedly loyal to Pazuzu. But the Hezrou could work, and it has a lot of abilities that the party is not used to having to deal with.
