How to replace banished cards

All:

So when you banish a card from your deck, how do you replace it at the "Between Games" step? If it is, for example, a "1" Spell card, and I have a Spell 1 upgrade, do I put it back into my deck before the next game? Or do I need to put a "B" Spell back into my deck and start over with it because I Banished it?

You replace as you would when building a new deck, that is:

AD 1 & 2:
Go in order like you would for your starting deck -- B Basic -> Any B -> 1 Basic -> Any 1 -> ... (only go to the next step if there are 0 eligible cards in the current step)

AD 3+:
Select any card with an adventure deck number at least 2 lower than the current AD.

You can also just use your upgrade to replace the banished card, which in your case means you can select that Spell 1 right back. That does count as your one upgrade for the scenario though.
