So, a question on how reporting actually works.
A character dies in a session, and the rest of the party succeeds in the scenario. The dead character's player gets to immediately start a new character, and apply one of the game upgrades to the new character. How is that tracked on chronicles (for the player) and tracking sheets (for the organizer)? I can't report the same player twice on the same tracker, but otherwise, their new character gets a deck upgrade that'll never be collected on a tracking sheet. Is the new character just reported on a single-player failure version of the first tracker? The example of reporting results really needs to include this.

For the chronicle sheet, just give them a new one with the new character number on it, write the scenario into that and have them register it. For the old character write the scenario as well and note that they died. The chronicle sheets are the source of record, not the online reporting system.
As for the tracking sheets, they're not really useful if you don't enter them into the online system (as Paizo uses those stats) so I assume you're meaning the online reporting system.
I fiddled around with it a bit and it looks like you can't report the same character twice. Just report the dead character then. Yes it means the online tracker doesn't track the new character's upgrade but as it is right now it can't track many things and they're working to make more things trackable.
What is unclear is if your new character completed the scenario if your old character died (the rules don't say). The part about your character not getting the scenario reward seems to imply that your new character doesn't complete it.

The new character does not actually complete the scenario nor actually participated in the scenario. You report the dead character on the chronicle sheet (and online). You make note of the upgrade on the new character's chronicle sheet. But not as a particular scenario upgrade at this point (because the character has not actually played anything yet.) You don't report the new character yet but you can register him/her.
When the new character has completed their first scenario, report the extra upgrade then. It's already noted on the chronicle sheet.
