Does anybody know where the 3.5 version of the grandfather plaque appears, or if someone has done a conversion. It originally appeared Dragon magazine #223, and reprinted in Monstrous Compendium Annual #3.
I'd say that coverts to a trap. (This is a PFRPG conversion, not 3.5. Should work for ya though.)

Trigger proximity; Reset automatic
Effect spell effect (magic missile x2, may select different targets, plus shout)

Well I didn't reproduce the flavor, for obvious reasons, but it was no more intelligent than today's traps that can, "activate only when certain kinds of creatures approach." (link)
Reasons (from the original text) that support it being a trap:
*Immobile
*Notifies creator (like alarm)
*Part of a door's locking mechanism
The only truly fuzzy part is its "limited intelligence," which, in game, manifests two ways: it can identify passersby, and it will attempt to get itself re-installed if removed (or if encountered on a ruined door). That sounds like basic trap functionality combined with a magic mouth spell.
Now, just because it's a trap doesn't mean you can't play it up like it's one of those door knockers from Labrynth. And if you want to preserve the party's ability to overcome it simply by doing enough damage: hardness 8; hit points 45; break DC 30.
