This one's almost a two-for-one...
A few years back I was running my group through the revision of 'Mud Sorcerers Tomb' When they entered the room with the strange altar and magic carpet, a few of the party members wisely decided to circumvent the carpet by hugging the walls of the room so they could get to the altar. Once they got there they took quite some time investigating the items on it. Eventually, one of my other players got bored and announced he was walking over to the altar as well. Right over the carpet. As soon as his foot his the ground I announced to the group that his character had vanished. Naturally this freaks the rest of the party out. They then start frantically debating over how to free their companion from the trap (he was visible in the carpet as a small figure drowning). After a few moments, another player declares "I'm going to throw the carpet into one of those braziers and burn it!"
"Oh, so you grab the carpet?"
"Uh..."
*Poof*
Even though they managed to free the second player, this really made my day, I tell ya! :)

Ran a 3.0 game shortly after it came out. Party faced off against some styrges (sp). The abrbarian tried to help in the removal of one that had latched onto the sorcerer... with his great axe. Sorcerer was level 3, the first attack missed the parasite and hit the sorcerer, putting him down to 0 hp. Second followup attack criticaled the styrge and plowed through the low hp beast to finish off the sorcerer.

I was playing a gnome assassin back in the 1st edition days. I had a ring of invisibility. So, I was following the party around, invisible, and happened to be standing by a door when it slammed open, hitting me and taking me to negative hit points. So, I bled out, invisible, and I don't even think the party tried to find me.

So, I bled out, invisible, and I don't even think the party tried to find me.
No real deaths in this one scenario but it's close to your situation:
The party, all 14th level, starts buffing up and gearing up to take on the ancient white dragon in its lair. The wizard decides to cast mass invisibility on everyone... great. We rush in, turns out the dragon can see all of us... and we can't see each other except for maybe the wizard. A combat of hilarious proportions ensues. Flank? uh oh... A teammate went down... where? oh crud. :-)The memorable death in another adventure, years later (long winded):
First level human sorceror, human cleric, and a dwarven inquisitor are locked in furious conflict with a group of flying reptiles. The cleric goes down and the inquisitor picks him up and withdraws into the jungle. The sorceror instead of following the same path goes through another path in an attempt to draw off the reptiles which worked great. He just can't move well enough in the jungle terrain to shake them off and he spies the edge of the seaside cliff.
He decides to jump off the high cliff (50') to escape the reptiles. Amazingly he takes just enough non-lethal damage that he's now at 1 or 2 hp above consciousness. Great, he's gonna make it! wait... the seas have a strong current, he starts making the swim checks. No go, he gets swept further and further out to sea, eventually having to make str checks which fails and he goes to fatigue and eventually to exhausted... uh oh. Of course no one knows what happens to him. Now I keep piping up at the table whenever we split up the group, 'and they never heard from him again...' :-)
