Activating the shoes is a standard action. Is the "lightning leap" part of that standard action or does it count as my move action?

Let me elaborate on my character. He is a duelist who uses an aldori dueling sword. I took a couple of levels of Master of Many Styles to add some more flavor to my character. I envision him being able to attack with his sword and throw a knee/elbow/punch, but am unclear on the game mechanics.

I have a fighter/monk. What are the rules concering combining unarmed and weapon attacks. In the monk description it states:

Unarmed Strike: At 1st level, a monk gains Improved Unarmed Strike as a bonus feat. A monk's attacks may be with fist, elbows, knees, and feet. This means that a monk may make unarmed strikes with his hands full. There is no such thing as an off-hand attack for a monk striking unarmed. A monk may thus apply his full Strength bonus on damage rolls for all his unarmed strikes.

The line that says there is no such thing as an off-hand attack for a monk striking unarmed is throwing me off. Do I still need the two weapon fighting feat to attack with both my weapon and unarmed?

If there is a fog cloud in effect and I'm in a square covered by the fog cloud and my adjacent opponent is not within the fog cloud do we both have concealment or only me since I am in the fog cloud and he isn't?

Can the style feats in Ultimate Combat be used when fighting with a weapon or only when unarmed?
