I guess in that previous example Critical Hits would simply add to the multiplier. So a super Fighter at Lv 20 with all the works hits for something like 170+ damage on a Critical Hit... invoking doom on his target. So... perhaps that isn't the best direction. @_@
Perhaps a die bonus for multiple attacks, like the previously proposed feat.
Maybe magical enhancement should improve the weapon's die... Either increasing the number rolled (1D10 to 2D10) or improving the actual die itself (1D10 to 1D12).

In any event, someone needs to do something at some point in some way that makes stuff happen.

I think a mid-level fighter only feat that increases his weapons die damage by 1 should be introduced (thus 2D4 becomes 3D4, that sort of thing). Boost damage a bit.
I also don't really care for multiple attacks. I think these additional attacks could be translated into a multiplier. This multiplier would be ignored when using a non-standard Attack Action, such as when performing Cleave or some such thing. Perhaps a Full-Round Attack simply boosts the multiplier by 1. I don't know, just random ideas off the top of my head. =D

Hmm...
I remember having a 15th Lv Fighter (some of those Lv were Pious Templar) and I ruled the world. Did something like 40 ~ 60 damage every Round, had DR, good Fort and Will, Fast Healing 4 for the 1st 10 or so Rounds... Using official Feats and equipment. =D

I sort of despise how the rogue rolls so many dice for Sneak Attacks in the first place... I think a solid bonus would more balancing and make the damage phase move faster. Having 10 ~ 60 points of additional damage plus Bleeding is pure insanity. I think I remember seeing a 3.5 Feat that increases the D6s to D8s. @_@
In any event that is a stupid amount of dice to roll. I think a solid bonus equal to the Rogue's Lv would be better. After that, add special options to make up for the lost damage. Better Critical Hits, larger Critical Ranges...
I guess I never made the mental changeover from Rogues: Masters of Skills to Rogues: Masters of Skills and Ninja-ing Folks Directly To Death. What is this, WoW??? =/

The only thing I hate the more than 3.5's magic system is the way all of the classes, feats and prestige classes are spread out through tons of supplements.
This is unavoidable... as fitting all of the preexisting 3.5 content into the core rules would create a massive book that couldn't be navigated well nor even published. So... Take time to consider the lazy folks as these supplements are being popped out. =D
Keep an up-to-date list online complete with book and page references. Several lists that only include titles as opposed to explanations, we aren't looking for free content after all. At a minimum, one of each for the following; Base Classes, Prestige Classes and Feats.

Do this and I'll give y'all money in exchange for your products. =D
Hey, that would be at least one sale in the bucket for very little additional effort, yeah???

[Random Thought] Oh, the additional options made available via Skill Tricks from the Complete Scoundrel should be in the Core rules. =D

I think that Prestige Classes could be removed entirely by the way... I don't have the time nor the patience to read through this entire thread so I'll lay my idea down and hope that no one else has suggested this already.
Prestige classes are more often than not simply alternative options for certain character types; warrior, arcane magic-user, divine magic-user, rogue, etc... In that spirit just modify the class features to react like Feats only they replace a class feature (so they don't actually consume one of the character's feats).
Every special option made available by Prestige Classes in 3.5 can be emulated by doing this. Assign each of these Alternative Class Features requirements such as Base Attack Bonus, Levels, Specific Class Levels, Base Saving Throw Bonus, etc... This way Players can mix and match options without muddying the character sheet up with different class levels.
