Was there a reason why Tier 4-5 chest has only 1 copper piece in it vs Tier 1-2 having 1600 Gold?

So I'm getting ready to run Curse of the Riven Sky for my group and I'm quite confused about the Book of Titan.

It says that it was a mission that the Pc's should have played beforehand to obtained but I have found nothing.
So my question is this, What is this mission where you would have obtained the Book of Titan?

So I'm running the Price of Immortality modules for a group and (spoiler?) the Jewel of Everlasting Gold can only be destroyed in the runewell of greed. But I can't seem to find it in the module but because the artifact can't be taken out of the city how can it be destroyed?
Where is Karzoug's runewell of greed?

I know it may not really matter to the story but I'm legitimately curious
