I've never been a TPK, or a single player, killer DM. Death's in my game tend to occur when one character chooses to separate from the party, then continue down a path after a warning or two. Although flirting in the negatives digits have been known to occur until healing. :-)

It took us 20 minutes to get the loot all organized, Chell could have had a prayer then.

Want to talk about a bychte of an encounter: My RA group just beat a 12 headed pyrohydra (a pet of the deity Hecate) which had a hellfire template that exploded once the hydra was destroyed, that triggered a curse from the deity Hecate which hit two members of the party with the panic condition, and 3 others with the shaken condition. Thankfully they had a paladin and his immunity because the curse also triggered the release of a Gelatinous Cube, Sorcerer that has been toying with the party for entertainment.

No deaths but frustration abounds since only one member of the party can see invisibility. The creature hasn't taken any offensive spells but did things like stone shape a wall to separate the party in two, then it dimensioned door once the party keyed in on it's location. I actually dropped the invisibility spell, so move the action forward.
