I think I found why the Chime of Opening kinda sucks at its job as mentioned before. It is based on the knock spell which uses CL + 10 to open things. The item is missing that +10 and it is probably a mistake, but one that is unlikely to ever get errata. I included the +10 in the rolls above and may have still failed twice :P.

Jod returns Hannik's cold iron weapon, thanking her for its use. It takes him a bit to get into the chainmail, but fortunately he has others to provide a little aid this first time.
As they prepare to open the door, he takes a knee in front of the door (in P8) so others can shoot over his shoulder if battle breaks out, prepared to stand and charge once the enemy is engaged.

Kira tends to take up a rear guard, which means she ends up shooting her crossbow a fair bit. But she's good for Melee.

We know that Mulluq would insist on being the one to open the door. Rhok doesn't seem to be in the formation, so I'll put him next to Mulluq.

Along one wall of this long room stands a smashed sideboard. A stone staircase leads upstairs. Three blobby fiends slouch on the floor but burble happily and get up as new victims present themselves.
You have seen these before - they are dretches. Please roll initiative. I'm assuming, since you didn't say but carefully set yourselves up in line, that the archers will have prepared ready actions to shoot at any hostiles. You can resolve those actions first.

Booam was ready to go, moving into the room just before Mulluq can get in there. Raising his heavy greataxe high, he hacks violently into the first of the dretches.

Mulluq also rushes in, but moves past the first one, to stop right in front of the second, both leaving room for his allies to enter, as well as creating a partial blockade to keep the enemy from advancing. He swings out with Radiance. His swing is pure, and delivers a decent blow to his enemy.
Move to L9, no provoke as they are still FF. If D3 lives, he targets that one, otherwise, D2.

It's fellows are taken down in a few seconds but the remaining demon gives no thought to fleeing, the demonic compulsion to kill and destroy being all it has to drive it. It lurches forwards at Mulluq.

The way I work that is to apply the -4 for firing into melee. It makes it easer to work out the penalties, whereas the whole soft cover thing can be a bit confusing. For example, if you miss, do you hit the soft cover (i.e. your fellow party member)? I don't really want to get into that, so I just apply a flat -4 for shooting at anything or anyone in melee with a PC. Note that Precise Shot makes that penalty go away.
Sol's shot is wide. But Rhok's demented exhortation makes it quail.

Although Hannik is ready with her crossbow loaded, the corridor is far too crowded to risk a shot. With only one demon left standing and it looking like the front line has the situation well in hand, she waits for now.

They sort of stack. Target being in melee is -4 to hit (unless you have precise shot) and an ally or enemy standing in the path of your ranged attack to the target is +4 to the AC of the target (unless you have improved precise shot).
EDIT: I may have misunderstood what you were saying. Do you count both conditions as -4 to hit so the PC can figure them into the roll or do you ignore the cover penalty from an ally being in the way?

Indeed, no cover penalty from allies.
There is the thump of feet on the stairs as three tieflings run down, aim crossbows and shoot.
T1, T2 and T3 open fire on Kira, Jod and Booam respectively. Due to height differential with the ground floor only T1 can be reached in melee for an AoO (by Kira, with penalties for cover). Also, if you want to melee with them, you will need to go round to the foot and stairs at J7 and go up (except for T1).
T1 1d20 + 3 ⇒ (16) + 3 = 19 to hit, damage 1d8 ⇒ 2
T2 1d20 + 3 ⇒ (11) + 3 = 14 to hit, damage 1d8 ⇒ 8
