I don't think so. If memory serves - Jason said that it would be one survey per person which makes me think that it will probably be sent out to the email registered to our Paizo accounts.

Hmmm... That's kind of annoying. I don't use that e-mail or check it unless I've ordered something. If that's how you get it, I'd have missed it for possibly months without seeing this post.
Grumble, grumble.... Now I'm going to have to keep checking it until it shows up. :(
