Gashadokuro (CR13) - enormous skeletons that come into being as a result of mass starvation.

The Trench Mist is a weird case; it's created by the death of lots of people in one place, and can create undead spawn of its own, but it's not actually undead. It's an aberration. Go figure.

From Monsters by Type. I think some of these are 3PP...
Undead raven swarm (Many undead, but not technically 'people')
Carrion Storm (Similar)
Corpse orgy (Another not-actually-undead.)

Any creatures of the nightshade subtype, though they are the product of the death of otherwise immortal Outsiders.
You could potentially add an undead template to any of the creatures with the troop subtype. Group undead could also be made from the Mob template in the (3.5) DMG II.
Other examples:
Caller in Darkness
Drocha Swarm (Wrath of the Righteous: Demon's Heresy, caused by mass deaths brought about by the opening of the Worldwound, CR 7)
Necrocraft (manufactured from various body parts)
Sea Bonze
Tzitzimitl (potentially)

Lol... that is indeed odd, the fact it's powers to animate Juju zombies and the other GE thread about the magical child archetype kinda makes me wonder what would happen if you made one of these a level 20 magical child as a boss...
