From the D20 website regarding weapon size:
"Every weapon has a size category. This designation indicates the size of the creature for which the weapon was designed.
A weapon's size category isn't the same as its size as an object. Instead, a weapon's size category is keyed to the size of the intended wielder. In general, a light weapon is an object two size categories smaller than the wielder, a one-handed weapon is an object one size category smaller than the wielder, and a two-handed weapon is an object of the same size category as the wielder.
Inappropriately Sized Weapons: A creature can't make optimum use of a weapon that isn't properly sized for it. A cumulative –2 penalty applies on attack rolls for each size category of difference between the size of its intended wielder and the size of its actual wielder. If the creature isn't proficient with the weapon, a –4 nonproficiency penalty also applies.
The measure of how much effort it takes to use a weapon (whether the weapon is designated as a light, one-handed, or two-handed weapon for a particular wielder) is altered by one step for each size category of difference between the wielder's size and the size of the creature for which the weapon was designed. For example, a Small creature would wield a Medium one-handed weapon as a two-handed weapon. If a weapon's designation would be changed to something other than light, one-handed, or two-handed by this alteration, the creature can't wield the weapon at all."
I don't know why I'm having such a hard time wrapping my head around these rules. Question is simple: Can a large creature use a huge 2H weapon, and if so, at what penalties? -2? And how would the Fighter [Titan Mauler] ability, Massive Weapons factor into this?
The class feature reads as "She can use two-handed weapons meant for creatures one size category larger, but the penalty for doing so is increased by 4. However, the attack roll penalty for using weapons too large for her size is reduced by 1, and this reduction increases by 1 for every three levels beyond 3rd (to a minimum of 0)."

Long story short. A large creature can't use a huge 2H weapon, since there isn't a size progression after 2H when wielding bigger weapons.
Light becomes One-handed
One-handed becomes 2H
2H becomes Unusable
(the reverse is true for big creatures wielding smaller weapons)
The penalty of -2 occur when you wield a weapon of different size than yourself, but is still able to use it.
For instance:
A large character wielding a huge One-Handed would be forced to wield it 2-handed (one-handed becomes 2H since) and take a -2 penalty.
A medium character wielding a huge Light weapon would also be forced to wield it as 2H (Light becomes One-handed, One-Handed becomes 2H), but at a -4 penalty.

So does this FAQ change mean that a Medium Titan Mauler can wield a Large greatsword with what total penalty? -4 or -6 (-2 for incorrect size + -4 for TM ability)

Where does it indicate that a Medium creature can wield a Huge weapon? Didn't think that was possible RAW even as a Titan Mauler.

The key word here is "each". This implies that there can be more than one size category of difference, if my interpretation is right.
Also, since wielding a huge light weapon as 2H isn't in violation of the last paragraph, I don't see why it wouldn't work.
If there is any FAQ ruling that contradicts what I just said, please let me know.

A large creature cannot wield a huge 2h weapon, the rules for that are quoted above me.
A large titan mauler can wield a huge 2h weapon.

Where does it indicate that a Medium creature can wield a Huge weapon? Didn't think that was possible RAW even as a Titan Mauler.
I believe wonderstell is correct, in that a medium creature can wield a huge weapon. But only a weapon that's normally "light" for a huge creature. And not any gargantuan weapons.
Whereas a large can wield a huge light (as a one handed), or a huge one-handed (as a two handed), but not a huge 2-handed (normally)
