I've been working on this document for personal use to make my sessions run smoother. It's a sheet to summarize the various encounters, their locations, 4-player adjustments and important monster stats (AC and Initiative modifier) for quick reference.
I know the formatting is still rough, but I thought I'd post this to see if anyone wants to use this or give me feedback on its usability.
I was thinking about making the back of the sheet an area to mark down reporting notes, such as keeping track of deducted gold, faction mission successes or secondary success conditions.

It's a nice summary, but I'd never use it, personally. When prepping a game, I want to have the entire stat block around, and I'd rather my prep document spans several pages in order to make that happen.
If it works for you, though, go for it. You might find that including the monster's hp along with Init and AC is useful for quick reference.
