A bit unclear if it detects all the listed creatures simultaneously, or if it has a range of choices by type but only detects one type per use.

The first line includes that "or" bit which has me confused with the second line's "all of the creatures" bit.

I don't understand where the confusion is coming from. Presumably you think that the line about "detect all, not just one kind" might mean something other than a direct contradiction of your "range of choices, one per use" idea, but if that's not what it means, what COULD it mean? I can't think of any possible meaning of that sentence other than to explicitly resolve the very question you're asking.

So, what are you imagining it might mean instead?

Semicolons indicate that there is a list within a list. The sub-list "astral, ethereal, or incorporeal creatures" is subordinate to the super-list "undead; fey; outsiders; etc."

A creature can be astral OR ethereal OR incorporeal and be included in the list of creatures spirit sense detects.
