Customized PFS Report Form

GMs and Coordinators:

I downloaded the PFS report form and built a modified form to suit our needs. It is a one page form. It has the cool PFS logo on the top, a spot for the six players, a spot for the optional seventh player and a spot for the GM to report their credit if needed. I added some of the Pathfinder artwork and faction logo's to it. We found it to be a little more player and GM friendly.

I built it in Publisher, but also have it in PDF.

Darn, it likely is too late for me to get one, and printed. I also took the form that is used and made three of them on the one page with blank event name and number areas for use at November's NeonCon here in Vegas.
Mind sending me a copy of the one you made? While I've likely had more than enough printed for NeonCon - it will be useful at Vegas Game Day events. Would you e-mail it to PFSTheocrat at gmail.com?
Of course, if Mark somehow makes it available that might work too!

Theocrat - I am coming to NeonCon and would be happy to bring plenty of NeonCon customized forms (in color!). Just say the word! I get into Vegas on Wednesday and am staying at the tropicana so I can get them to you well before the con starts. Let me know!

We used the three session single page form at our last convention, but the only problems we had is that if a table only had five players, the next session started at the open spot from the first session, so we had to watch out for sign up errors. Then there was not a spot for the mutant seventh player and no where for the GM to list his credit if applicable.

I received a handful of e-mail requests, so they will all be sent out this morning.

They look great! Thanks for your work on these. My father-in-law lives in Denver and we usually out there for a week or two every year. Maybe in 2011, I can help you run some games, perhaps the Venture-Captain special when it is released :)

I think the wife wants to look at going out during my 15 year old's spring break or early summer. So, chances are we will be that way in March or June. I'll make an announcement if andwhen plans are firmed up in a few months.
