Ei'risai, the Slaadi equivalent, though they're more like the 2e Hordelings in that they have a whole bunch of random appearence tables.
Evil Eye, the Beholder analogue, a giant floating eyeball with trailing optic nerves and blood vessels and such
Greymalkin, a cat from another dimension that can access this one at odd angles (Displacer Beast)
Ophiduans, your standard Yuan-ti snake people, with a nifty psionic twist (they can implant a severe fear of snakes into someone's mind.)
Phrenic Scourges, your mind flayer replacements, though these are significantly creepier since they implant you with their young and turn you into one of them at any time, rather than in some grand ceremony back home. (These actually got a bit of 3rd party support from Dreamscarred Press here)
Scavenger Worm - your Carrion Crawler counterpart
Tunnel Brute - Umber Hulk with a tail stinger, much smaller mandibles, and a despair aura rather than a confusing gaze.

I really dig these creatures. Personally, Beholders and Mind Flayers were getting a little played out in my campaigns, so these guys were a fun twist on similar themes. The Greymalkin and Scavenger Worm aren't quite as creative as their D&D counterparts and the Ei'risai are a little too chaotic, but the Evil Eye, Phrenic Scourge, Ophiduans, and Tunnel Brute are all great. I especially like the twist in Ophiduan society that puts the human-breeds at the top of the hierarchy.
