No.

You only have two hands (assuming a normal PC race). A main hand and an off hand.

You can make an attack with each hand during TWF.

Even if you are attacking with a weapon that doesn't actually use a hand (like a Barbazu beard, bladed boot, or head butt unarmed strike) it still counts as using a hand, because it is either a main hand or off hand attack.

So if you use b. knuckles in your main hand and b. knuckles in your off hand, you have no more hands to use an unarmed strike (even one with your head).


Yeah otherwise monks would be attacking 5 times (punch punch kick kick headbutt) and that's not even including elbows and knees.

Depends on your question.

If you had two attacks (BAB +6) and TWF, then you could attack with one Brass Knuckle attack for 1d3, the second attack with your other Brass Knuckle for 1d3 and a Two Weapon attack with your Unarmed Strike for 1d6 or whatever your unarmed attacks deal.

No. Critters get more attacks from more limbs. Human beings don't, its why you can't fist fist elbow elbow kick kick headbutt bite bellybump buttbump someone.


Only if you are a monk - and they can only specifically use any body part to attack, however it's only flavor in that the number and damage never changes.

That is you get your flurry (3 hits) and can say 'I hit with my head' - but it is still 3 hits total at the end of your attack and the damage doesn't change.

Anyone can use any body part to attack. The combat chapter defines an unarmed strike a a fist, kick, or head butt. It is not monk exclusive.
