I think my favourite items are those that are simple and can be made into props around the gaming table.
ANyone remember those necklaces made of sweets - theres your necklace of fireballs.
Prayer Beads - a bag of marbles does the trick.
I like unusual and gimmicky stuff like that.

Oh, for things I've really wanted, to use, but never did get to (partially because I GM all the time these days) are, like some others have said, the Marvelous Pigments and Lyre of Building.
For things you always use, the Handy Haversack, as mentioned, and once available the horrific, wonderful and super scary (I've seen it used for murder 3 times by overly creative PCs experimenting with evil-ish characters) Portable Hole.
Oh, and the in one of the other book there was this Wasp/Asmodean themed corset - I believe it was in the Cheliax Companion. That was neat, very neat. I want to make an NPC wearing it, just to get to use it.

as a player: Robe of Useful Items and Handy Haversack
as a GM: I love giving my players a Deck of Many Things(even 'tho it's technically an artifact now) and watching them agonize over whether or not to draw from it.
I've done something like that, but more devious.
The PC's entered an orc gambling den, one of the tables at the back had a deck of many thinks dealt face down in hands of stud poker. (The orcs in my campaign of conivinng nasty ideas orcs.)
Yes, they looked before detecting magic.
It was... bedlam.
Grin.

I have to go with Feather Token Anchor and/or Bag of Tricks (Tan) for personal stories with them.
Both are somewhat beyond the normal set of 'rules' but what else are wonderous items for?
The anchor tokens are really fun to fletch arrows from especially in naval combat.
Bag of tricks Tan... I feel so bad about this but we were exhausted at the end of a long session and just trying to find a safe place to stay. Stuck in a dungeon, traps in the way that we had basically run out of ways to get around. Decided to just trigger them with animals from the bag of tricks... poor poor grizzly bear... one of the funniest DnD stories ever.

most of my players would've just (attempted to) slaughter the orcs for being orcs and would've ignored the "playing cards", but i can see how that would have been cool with a different group.

It's 3.5, but I like the Belt of Battle. A small boost to initiative, and the potential for a variety of different types of extra actions in a round (move, standard, or full round) a very limited times a day lets you have fun with all sorts of action economy AND resource management.
Boots of Speed are nice too. An extra attack or 30 feet boost of speed 10 times a day is nice.
Hmmm....I just realized that 3.5 Spell Compendium ranger spell, Swift Haste, is pretty much the spell version of Boots of Speed! Neat!
Necklaces of Fireballs are a fun way to give some bang to someone who usually bashes.
