Just a minor question to Fatespinner who started this thread, Does the character have to have stayed dead? Or can we post in deaths of characters that eventually brought back via some kind of magic?
The only qualification is that the character must have been DEAD. If they come back later, that's just the nature of D&D. So yeah, if they come back, post here anyway!

Circumstance of Death: After deciding to leave the rest of the group behind for some sneaking around, he actually wanted to open the gates of a city being held by the bad guys so that the army of good could come in and retake it. Longer story short he tried climbing the wall with 3 Fire Giants each with at least 1 Hell Hound standing near him. 2 of the Giants hit him with their axes and 1 of the Hell hounds breathed on him in the same round and same init, making him a nice pile of flaming goo.

Circumstance of Death: After being knocked down to only 4 hit points, this character proceeded to cast Sanctuary on himself then he moved in between the Fighter character and a raging Ogre Barbarian. Keep in mind the Fighter still had most of his hit points and because he was using Combat Expertise was doing a good job. Needless to say the Cleric was turned into Elf Jelly by a giant sized great club. The other pcs were thankful because the impact of the club saved them the job of digging a grave.

Circumstances of Death: Defending a dwarven stockade/mine against a marauding band of orks, Ork Sniper (ranger 6) abseils down the cliff face at the back of the stockade in the darkness, shooting at the illuminated defenders. WIzard turns around , climbs up ladder onto stable's roof, gets narrowly missed by arrow the monet he pokes his head over the edge. Next arrow grazes his shoulder... Wizard pulls his head down, no more arrows, waits two rounds, no more arrows, pokes his head up again, assuming the sniper to have moved on. Sniper ("Herger" who became an infamous nemesis in the campaign ) rapid shoots him, scoring a double hit, one a confirmed crit for a whopping 39 points of damage... wizard expires with an arrow poking out of each eyesocket.

Circumstances of Death: pursuing the retreating orks to their base after a successful (if bloody) defense, the players catch up with the bad guys in a narrow gorge, with heavily wooded slopes and eroded ridges. Skymage flies ahead on his flying mount, hovering/circling over a ridge on side of valley for 30 (!) minutes, while casting "clairvoyance" to spy on orcs.
Orc Sniper ("Herger" again ) leads his remaining scouts/snipers up the hillside and has them climb ridge ( Skymage, locked in clairvoyant trance does not notice ). Orcs fire a sniped salvo at 90' hitting the mage with a couple of arrows, one of them a poisoned arrow from Herger. Skymage survives, even makes (DC 15) initial Fort Save, flies back to group, fails second Fort Save 1 round before reaching group (and safety ), loosing 6 points of CON and 21 HP in one fell swoop and drops off his mount, dead, 50' away from his fellows.... He did not carry a healing potion =)
The player was extremly piXXed by now...

Circumstances of Death: Defense of above stockade, Kensai, covering the retreat, gets himself cornered in a storage shed, breaks free and runs (!) for safety - pursued by two worg-mounted orcs and 2 more worgs. Heads for the nearby mountain lake (ice cold, spring ), worgs trip him 20' into the lake, goes under in 4' of water and never gets up again, from a combination of drowning, being chewed to pieces by worgs and eternal shame.

Circumstances of Death: in pursuit of a large group of gnolls and fire giants who had recently killed the Bishop of Azuth/Velsharoon (FR conversion of Wee Jas ) and kidnapped the mayor of Cauldron (a "big-ruse" side adventure ), the PCs come to a canyon blocked by a crumbling orcish stockade, appearantly deserted. Suddenly sniping fire from the canyon's flanks strikes the group. Jacob gets himself dimension-doored up to the rim (by his friendly mage), slays two orcs... Hears a wicked laughter from over his shoulder, turns around and spots "Herger" the orc sniper ( by then a level 11, Ranger/Rogue/Deepwood Sniper ), 90' away... bellows a challenge and charges. Herger fires three arrows, three hits (with a whopping damage modifier of +13 for each arrow), one of them a crit for max damage of 21x3 points... the blackguard, barely alive, is forced to roll for massive damage, and fails with a natural "1"... Corpse falls sideways and looks very stupid with an arrow sticking out of its mouth...
By then, they had really started to hate "Herger"

Circumstances of Death: A church agent send to identify and possibly eliminate the newly-formed vampire engages her in the characters' lodging's gardens. A rough and ready tumble ensues, and they grapple each other. Rest of group intervenes, and the Favoured Soul of Kossuth sends a ray of sunlight into the grapple - rolling a natural "1" to hit, and since we play with fumble rules, confirms for effect, rolling "worst case" (basically a 19 ). Strikes the vampire, pulverizing her with a single spell re rolled like 45+ points for damage.
Charboiled vampire expires, chargrined player mumbles a low-key "sorry".... I did not even have to touch a die for that take-down =)

PC's Name: Naomil - the good spirit of Calladyr
Race: Gloaming (FR Underdark )

Circumstances of Death: Trapped in Occipitus ( I actually had the player enter there at the end of the previous adventure, getting trapped in the demi-plane to facilliate their participation) and about to face the Test of the Smoking Eye, the PCs meet up with an NPC Wizard Karras, who has been trapped here, and serves as a deux ex-machina to replace the usual "traitor-NPC".
Since he is a hard-to-like outsider, Naomil takes a sudden, brutal and intense dislike to him ( I somehow suspected the player to have read the actual adventure with regard to this , expecting Kaurophon) and hits him with a sudden "feeblemind". Karras saves, wins the initiative and pops up a Globe of Invulnerability. The other PCs, rather surprised and actually looking for allies and information, hold their blades. Naomil bombards Karras for another three rounds, while he dispels her and/or let her spells fizzle on his shield. On the fourth round he tires of her tantrum sighs, and hits her with his disintegrate.... a short "poof" and some fluttering gloaming wing fragments later, the spat is over.....

Circumstances of Death: Catching up with the competition for the Test of the Smoking Eye, the rakshasa and the fire giant , who I had beefed up slightly to compensate for the PCs increased (level 11-12 at this point ) and numbers 6 chars instead of 4, the Rakshas now having 9HD and corresponding magics (sorc 9 instead of 7) and a Mummified sorcerer (4th level) as a cohort.
PCs, making lots of noise, finally pick up the noise of casting and of steel being drawn over stone from in front of them.
Styx, clinging to the shadows of the corridor moves ahead, reaching the chamber ahead.. The fire giant is illusioned as the basalt statue of a fire giant, the mummy (dressed in a full, hooded rope and not readily appearant as undead ) and the rakshasa are invisible and biding their time. Styx sneaks in, and seeks the "cover "of the fire giant statue..... he gets utterly surprised by the FG hitting him with his greatsword, barely escapes through a "shadow jump" to join the party and has himself patched up some, while everyone else engages the opposition. Styx, feeling awkward about the fire giant, tumble past him and engages the "caster" in the back aka, "the mummy" but coming face to face, he has to roll for paralysis from fright... and fails miserably. The mummy rips out his arms and batters him to death with them...
Again I had the sudden feeling that the player expected sonmeone else inside that robe...
Player glares at me, and sulks...

Circumstances of Death: Test of Sacrifice,
After having beaten the final mummy, they are now contemplating his words ( I had these engraved on a stone Pillar too, just to make sure they got the final message, lacking Kaurophon =) ). PCs look about, and , being mostly despicable specimen ( NE Cleric, LE blackguard, LN/E Favoured Soul (Kossuth) , LN Wizard and a CG/N bard+ anything), eye each other. Tension ! Drumrolls !! Ominous Thunder !!!
The only thing missing is the theme from "once upon a time in the West"
The NE clerics player leans back and mentally starts drawing up a new character.... And everyone pounces on the loudly protesting aquatic elf, grappling him and tossing him into the plasma torch...
Next thing, the favoured soul tries to toss the blackguard, the wizard (low on spells ) goes invis and tries to hide etc. etc... total chaos for a couple of rounds
This almost grew into a TPK by their own hand...
We switched campaigns after this for some months, to let tempers cool down, hehe

Circumstances: Faced off with the rest of the party against an undead sorcerer and his 5 skeleton minions. Sorcerer used invisibility and struck Rundali with a ghoul touch while he was engaging one of the minions. Failed the save, sorcerer moved on, giving a command to the minion to "finish him." Minion obliged with a coup de grace.

The bard and wizard rained arrows and crossbow bolts on the sorcerer in an attempt to hamper his spellcasting, but piercing damage is not terribly effective against skeletal undead...

TPK!!!
PCs: Kithrain, Rundali, Altmose, Nightengale, Boran, and Jekal.
Races: All elf except Boran who was Human.
Classes (in order): Ranger, Rogue, Wizard, Bard, Cleric, Barbarian
Levels: All 6 except Boran (4) and Nightengale (7).
Circumstances: Yes, this is the same party as my last two entries. They just didn't learn.
The party was in a dark elf temple hidden beneath a dense forest and had found their way into the dungeon area of the complex (where they had just rescued Boran). They were trying to get back outside after their entrance tunnel had collapsed due to some very spiteful dark elf wizards who were about to die. Eventually, they found a dead-end hallway with a six-armed statue of a dark elf female at the end. Each of her hands held a different weapon (morningstar, flail, longsword, scimitar, sickle, and warhammer). While Rundali was searching the statue and the wall behind it for secret doors and traps, Altmose used detect magic on the statue. He learned that each one of the weapons the statue held was magical. Rundali completed his search and, upon discovering no traps, decided that the magical weapons could be of great use (since the party had NO magical items whatsoever at this point). He grabbed the scimitar and handed it to Kithrain. At once, they heard the sounds of a massive stone slab sliding into place in the direction they had come from and saw a small doorway open up on the side of the hall. Panicking, Rundali grabbed the other weapons and distributed them quickly, releasing FIVE MORE of the side doors. Behind each side door were 6 skeletons contained in a tiny room. They poured into the hallway and, with the stone slab and the dead end preventing any escape, 36 skeletons quickly put a violent end to the entire party, especially since the wizard had already blown his only fireball earlier that day...
Setting: Ravenloft, Homemade campaign
Man, I loved that campaign. They didn't get resurrected from this one. It was a game ender.

PC's Name: Rigun "The Fishmonger" Mor

Level: 10
Circumstances of Death: Grappling with a vrock. Party comes along a deserted village following a fleeing Iuz army. Of course, Iuz priests have left a few "suprises" for persuers. This village has 2 vrocks in it that know of teh approaching party. They commence in 'the dance of ruin' after succesfully summoning a third vrock and casting mirror image so it looks as though there is about 12 vrocks instead of just 3. The dance goes off - all party making saves except Fishmonger and the bard. The bard is at -6 hp and Fishmonger is still up at 2 or 3.
Fishmonger casts cure moderate wounds on himself and (already buffed with divine strength and righteous might) moves in to help the bard. After that, one of the vrocks shows up to finish them off, Fishmonger and the bard (now up and ready) hiding behind a stone arch. Melee ensues. The bard is back in negatives again and Fishmonger is down to 8 hp while the vrock is at 9 hp. The enlarged and enhanced cleric decides to initiate a grapple to keep the vrock from going back into the village to help the other vrock fighting his company members. In the grapple, Fishmonger actually even pulls out a mw kukri gifted to him by some Flan militia members. The fight goes for about 2 more rounds with no damage being done to either Fishmonger or the vrock. Finally, the vrock gets a claw in and it is a crit, Fishmonger is at -9hp. Fishmonger goes down and the vrock spores sends him to -12 hp by the end of the round.
The vrock is slain immediately by the dwarven assassin next round with a hit from his heavy crossbow.
Setting (and Adventure Path, if you're using one): Vagabonds of the Standard, Greyhawk 594cy, Rookroost/Redspan, homebrew.

Circumstances of Death: The group came up with a plan to ambush an insane cult member; a cleric that had been spurned even by his own insane collegues due to his behaviour. Farador then decided half-way through the ambush to change the plan. Several rounds later he was facing the cleric on his own, (with the other PCs getting their asses handed to them by another creature that they needn't have faced) and died with a mace being applied to his head, several times in quick succession.

Level: 4
Circumstances of Death: Learnt the hard way that fighting aginst a group of clerics is good enough reason for caution, especially when they're healing each other during the course of the battle. But getting seperated from the party and succumbing to slow melee death when surrounded was the defineable killing blow.
Even more delicious was when the party took A'Lan's body with them after the battle, and went searching for someone to Raise him. On their jouney they encountered the Troglodyte Cleric that had killed A'Lan in the previous battle, and had been the sole NPC to escape from the fight. Drawn into combat against skeletons the PC's left A'Lan's body on the ground, only to turn around and find the Trog Cleric standing above it and threatening to raise the body as a Zombie unless the party met his demands. (They did)

Saved against the first bolt, but then failed his reflex save against the second one, taking 21 hit points of damage, but only having 10 hp....

well im a DM about 98% of the time but i think the best death ive witnessed so far was the death of one of my own characters

so this was a while ago and i picked all of my spells to go off of my immunities.
so anyway all my spells were air cold and electricity based and in the campaign we were going into a fire world type deal and before the game that day i said to my girlfriend at the time that was playing too "its a good day to die" and everyone was like WTF they were like ok so we just enter the world and i tell all the other player to go that i would handle them, all of my big spells would of killed the creatures
but keith (damn him) convinced my girlfriend to stay there and help me so all of my upper spells would of killed her too. so 1 to 3 level spells only. kinda screwed me and he described the monsters that we were fighting as the fireball things from final fantasy and so i took out 5 of them without incident but on the last one when both of us were down to minimal points i cast hold monster so she could take it out
his response was
" well i didnt want to do this...but know how when those things are brought down to only a few hit points and they explode well that is basically what happens."
now mind you i was working on that chr for at least a year and a half. needless to say i really didnt enjoy playing with him anymore

they then said one by one

he stayed...he died. as three stories of tower fell on them due to acid damage in the fight before.

Circumstance of Death: During a mass battle the pc wanted to get to the commanding officer and his group, and let the npc's and the generic armies deal with each other. Fine by me. I had them each PC that made the attempt to get there make a couple of rolls, like a Dex check, and 1 saving throw. This was to see if they were hit and thus damaged by anything on their way, everything from a misc. soldier getting a lucky hit, blast from some spell. Beruk ended up taking 2 points of damage from dust in the eye. When he gets to the enemy commading officer he charges him despite the fact the commanding officer has a prismatic shpere up. After rolling the saving throws for each of the colors most of wich deal damage to you even if you do save, Beruk ended up dead the earlier dust damage being the difference maker.

Cataylst: ray of disintegration.
While atttempting to stop a cyst of Kaorti sorcerers and wizards from summoning an elder evil from the far realm, Samuel was hit by a Kaorti's disintigrate spell. He rolled a low Fort save, and took 120 or so damage. This killed him instantly.

Circumstances of Death: Hubris. After his 22 AC kept him relatively safe for a goodly portion of the adventure, bold Cery took to being the party's point man. Then he met a few gnoll barbarians with greataxes; +12 to hit and 1d12+10 damage made short work of the wee halfling.
Setting: Shattered Gates of Slaughterguarde, set in Faerun.

Classes: Fighter 13/dwarven defender 4, fighter 16, monk 15
Deaths: All petrified by medusa's when Druss decided to pick up a tempting skeletal arm with an iron ring on one finger. This released four medusa rogues who almost killed Druss in one round with sneak attacks. The surviving PC's fled (though not before Alwen fell into the pit trap, see previous post).

The party was camped in the Archenwood and attacked by a single troll. The troll surprised the watchman, rampaged through one of the tents, and flushed out the unarmored cleric. Claw hit, claw hit, rend and bite hit for max damage. Killed the luckless cleric before he hit the ground. It was the first encounter of the first night of a new campaign.

The party had beaten an ogre mage badly earlier in the dungeon, but allowed him to escape. The next day, they went onward knowing that the ogre mage was somewhere around. The party's warlock went ahead of the group to perform a recon of a sea cave using his spider climb ability to stay high up on the wall to avoid any nasties that may lurk in the deep and colorful tidal pool that filled the cavern's floor. The ogre mage turned invisible, flew to the warlock and attacked him with his greatsword but missed. The overconfident warlock decided to go toe to toe with the ogre mage and blasted him. Frustrated with his inability to hit the warlock, the ogre mage grappled him off the wall and flew to the center of the sea cave. He bellowed, summoning the advanced sea cat that lived in the depth of the chamber, then casually dropped his pet "kitty" a snack. The Huge sea cat did over 70 points of damage between its crits and rends, ripping the warlock into shreds before carrying his broken body back to its lair to feast upon. The sea cat was so large and savage that the party left the warlock's body in the lair, unwilling to risk any more lives to recover the warlock's handy haversack and the party treasure therein.

Circumstances of Death: She moved outside of the party's antimagic shell as they fought Sruggat, the worm naga sorcerer. Well aware of the shell and its source, Sruggat moved away and cast finger of death on her.
