I think you have Kr. Koob and Lapodie mixed up in your init tracker. Lapodie popped an image and already acted, Dr. Koob still needs to make a Reflex save, etc.

Now solo, though sickened from the fumes, Castana unleashes! REF: 1d20 ⇒ 16. Even dodging the worst of it, the explosion takes its toll on the man.
Zark fairs well against the fumes and the blast. He shoots and again misses the woman! But when Dr. Koob hits her with his special cocktail, she slows down just enough to make her an easier target.
Ears... It's hard to tell what he was aiming at. Still the fumes don't seem to bother him.
Lapodie has a bad go of it for a bit, but isn't out of the fight!
